Nd∶YAG激光两种切开方法治疗儿童后发性白内障的临床研究

目的:观察Nd∶YAG激光两种不同切开法对儿童后发性白内障术后瞳孔区障质或囊膜碎片残留的影响。方法:将后发性白内障患儿分为单纯组(单纯后囊增生型)20眼、张力组(张力性后囊下皮质残留型)20眼、无张力1组(无张力性后囊下皮质残留型)20眼、无张力2组(无张力性后囊下皮质残留型)20眼。对前3组患儿采取Nd∶YAG激光"十"字切开法切开后发性白内障,无张力2组患者采取"Q"形切开法切开后发性白内障并在术后应用安妥碘肌肉注射治疗。术后对患儿进行裂隙灯检查。结果:前2组患者术后瞳孔区基本无膜性或障质碎片。第3组患者术后瞳孔区残留较多的膜性或障质碎片。第4组患者瞳孔区残留较少的膜性或障质碎片。结论:应根据儿童后发性白内障的不同类型采取不同的激光切开方式进行后囊膜切开。

Clinical study of the posterior capsulotomy by Nd∶YAG laser in children with after cataract

AIM:To investigate the effects of two kinds of Nd∶YAG laser incision on after cataract in children.METHODS:Eighty eyes with after cataract were equally divided into four groups including simple group(only with posterior capsular opacification),straining group(with straining sub-posterior capsular opacification),non-straining group 1(without straining sub-posterior capsular opacification),and non-straining group 2(without straining sub-posterior capsular opacification).Performing a "十" style posterior capsulotomy with Nd∶YAG laser were undergone in frontal three groups.A "Q" style of laser incision and drug application was chosen in the last group.All patients were inspected with a slit lamp after the operation.RESULTS:Fragments of neither membrane nor the remaining cortical materials were found in the pupil area after operation in frontal two groups.More remaining materials were found in the third group but less in the last group.CONCLUSION:Laser incision styles should be chosen according to the type of after cataract in children,and some appropriate drugs should be applied after the operation if necessarily.
