摘    要:目的 通过观察胃癌和胃癌前病变中的幽门螺杆菌 (Hp)感染情况 ,以及增殖细胞核抗原 (PCNA)、抑癌基因 p5 3、癌基因Bcl 2的表达情况 ,探讨Hp感染在胃癌发生发展中的作用。方法 采用尿素酶试验、Warthin starry银染色检测Hp ;采用免疫组织化学SP法检测 5 3例胃癌、5 6例胃癌前病变和 10例大致正常胃粘膜组织中增殖细胞核抗原 (PCNA)、抑癌基因 p5 3、癌基因Bcl 2蛋白的表达情况。结果 胃癌组Hp感染率为 5 6 .6 % ,胃癌前病变组Hp感染率为 6 6 .1% ,均显著高于正常胃粘膜组(2 0 .0 % ,P <0 .0 1) ,而胃癌组与胃癌前病变组Hp感染率无显著性差异 ;胃癌和胃癌前病变组中 ,Hp阳性患者Hp感染与PC NA、p5 3、Bcl 2阳性表达及阳性分级呈正相关 (P <0 .0 1) ;在胃癌和胃癌前病变中 ,Hp感染与多基因共表达有相关性 (P <0 .0 1)。结论 Hp在胃癌发病中的重要作用已引起广泛的关注 ,但其确切机制尚不明确。本研究结果提示 ,Hp感染者具有更多的肿瘤生物学行为 ,包括胃粘膜上皮细胞增殖加速、抑癌基因p5 3的失活及癌基因Bcl 2的激活 ,Hp可能是这一演变过程的促进剂。Hp感染在胃癌发生发展中起一定的作用

Abstract:Objective To evaluate the effect of Helicobacter polyri (Hp) infection on gastric histology, cell proliferation, p53 and bcl 2 expression, and to explore the role of Hp in gastric carcinoma. Methods Hp was examined by rapid urease test and Warthin Starry method, and PCNA, p53, bcl 2 protein were examined immunohistochemically in 53 patients with gastric carcinoma, 56 patients with pre malignant lesions and 10 controls.Results The Hp infection rate in gastric carcinoma(56.6%) and gastric precancerous lesions(66.1%) were significantly higher than those in the controls(20.0%, P <0.01) . There was no significant difference in Hp infection rate between gastric carcinoma and its precancerous lesions. Hp infection correlated well with expression of PCNA, p53 and Bcl 2 in gastric carcinoma and its precancerous lesions( P <0.01). The co expression of PCNA, p53 and bcl 2 in gastric carcinoma and its precancerous lesions were higher in the presence than in the absence of Hp infection. Conclusion Hp infection could induce gastrc epithelial proliferation, inactivation of tumor suppressor gene p53 and activation of oncogene bcl 2. Hp might act as a tumor promoter in the genesis of gastric carcinoma.
