碘-125内照射粒子支架治疗中晚期食管癌的疗效研究

目的:探讨捆绑碘-125内照射粒子的食管支架在治疗中晚期食管癌中的临床应用和疗效。方法:35例食管癌患者分为两组,A组放置普通自膨支架,B组放置捆绑碘-125粒子的支架进行治疗,术后随访并发症、治疗效果及生存期。结果:35例患者支架置入过程顺利,位置合适,术后吞咽困难症状明显改善,连续随访未出现穿孔、出血、移位等并发症,生活质量良好,普通支架组平均生存期为120d,碘-125粒子支架组平均生存期为370d,明显高于普通支架组(P<0.001),2组术后血象和体重改变无显著性差异(P>0.05)。结论:捆绑碘-125内照射粒子的食管支架置入可明显改善中晚期食管癌患者的吞咽困难症状,同时对肿瘤行组织间放疗治疗,提高患者生存质量,延长生存期,疗效明显优于普通支架,且安全可靠。

Prospective trial of Iodine -125 seeds on middle and late stage esophageal cancer

Objective: To investigate the curative effect of Iodine-125 seeds on middle and late stage esophageal cancer.Methods: Thirty-five patients with T3 or T4 of unresectable esophageal cancer were allocated into two groups.The patients of group A were implanted into a covered self-expandable metal stent and of group B were implanted a self-expandable metal stent and I-125 seeds.After installation of stent comparison of the rates of successful stent installation,complications and survival time in these two groups were observed.Results: All thirty-five stents installation were successful.Dysphagia were improved in two groups.There were no complications of massive hematemesis and stent migration in two groups.The mean survival time in covered self-expandable stent group was 120 days less than those in group B 370 days(P<0.001).After implanting changes of blood WBC level and the weight between two groups had no difference(P>0.05).Conclusion: Indine-125 stent not only improved the dysphagia of unresectable middle and late stage esophageal cancer but also exerted brachytherapy,therefore,prolonged obviously the surviving time of patients,and its curative effect is prominently better than common self-expandable stent.And it's very safe.
