舒芬太尼用于甲状腺手术的剂量探讨

摘    要:目的探讨不同剂量舒芬太尼用于甲状腺手术的应用效果。方法将120例择期行甲状腺手术的患者随机分入Ⅰ组、Ⅱ组和Ⅲ组,每组40例。于术前5 min分别给予Ⅰ组,Ⅱ组,Ⅲ组舒芬太尼0.1,0.2,0.3μg/kg静脉注射。比较两组手术开始(T1)、术中牵拉甲状腺时(T2)及缝皮时(T3)血流动力学、镇静程度及镇痛评分的差异。结果Ⅱ组和Ⅲ组T1及T2时点平均动脉压(MAP)、心率(HR)显著低于Ⅰ组(P<0.05);在T1、T2及T3时点,Ⅱ组和Ⅲ组视觉模拟评分(VAS)及Ramsay评分显著优于Ⅰ组(P<0.05);Ⅲ组不良反应发生率显著高于Ⅰ组和Ⅱ组(P<0.05)。结论 0.2μg/kg的舒芬太尼持续输注镇痛、镇静效果好,不增加不良反应。

Abstract:Objective To explore the effect of different doses of snfentanil used in thyroid surgery. Methods 120 cases undergoing elective thyroid surgery were randomly divided into the group Ⅰ, Ⅱ and Ⅲ with 40 cases in each group. The group Ⅰ, Ⅱ and m were given sufentanil 0. 1, 0. 2, 0. 3 μg/kg by intravenous injection at 5 rain before operation. The hemodynamics, degree of sedation and anal- gesic score at the operation beginning(T1) , thyroid traction during operation(T2) and skin suture(T3) were compared between two groups. Results The mean arterial pressure(MAP) and the heart rate (HR) at Tl and T2 in the group Ⅱ and Ⅲ were significantly lower than those in the group Ⅰ( P 〈 0. 05); the visual analogue scale(VAS) and the Ramsay scores at T1, T2 and T3 in the group Ⅱ and group Ⅲ were significantly superior to those in the group Ⅰ (P 〈 0. 05); the occurrence rate of adverse reactions in the group Ⅲ were significantly higher than those in the group Ⅰ and Ⅱ( P 〈 0. 05). Conclusion The continuous infusion of sufentanil 0. 2 μg/kg has better analgesic and sedative effect without increasing adverse reactions
