股骨近端髓内钉在股骨转子周围骨折中的应用

目的探讨应用股骨近端髓内钉(PFN)治疗股骨转子周围骨折的疗效。方法应用PFN治疗股骨转子周围骨折48例。结果44例经4～18个月随访，平均11个月，所有患者均获得骨性愈合。术后骨折延迟愈合伴股骨颈螺钉退出1例，出现股骨颈螺钉退出、同时髋螺钉向内侧切出1例。根据Harris髋关节评分，优良率达90．9％。结论PFN治疗股骨转子周围骨折具有手术创伤小、固定可靠，应力分布分散、防旋转功能，可早期功能锻炼，其并发症可通过提高手术技术避免，是固定股骨转子周围骨折较理想的内固定物。

Treatment of Intertrochanteric and Subtrochanteric Fractures of the Femur with Proximal Femoral Nail

Objective To study the clinical outcomes of treatment of intertrochanteric and subtrochanteric fractures of the femur with the proximal femoral nail(PFN).Methods Forty-eight cases of intertrochanteric and subtrochanteric femoral fractures were treated with the proximal femoral nail.Results Forty-four cases were followed up for 4 to 18 months,averaged 11 months.Fracture union was obtained in all the patients.The local complications included one delayed union,one exited femoral neck screw and one implant cut-outs.According to Harris criterion,excellent or good rate of the function of the hip joint was 90.9%.Conclusion PFN is a useful device in the treatment of peritrochanteric fractures of the femur.It is relatively easy with less surgical trauma.It has the advantages of anti-rotation,stress-dispersing,rigid fixation and allowing early functional exercises.The complications can be avoided with promoted operative technique.
