盐敏感性高血压和非盐敏感性高血压对靶器官的损害及护理对策

摘    要:目的 探讨盐敏感性高血压和非盐敏感性高血压对靶器官的损害及护理对策.方法 选取2009年3月~2013年2月就诊的高血压患者116例,依据盐负荷试验结果将患者分为盐敏感组和非盐敏感组各58例,将两组患者再各自随机分为A、B两组各29例.A组在常规治疗的基础上给予以针对性护理对策进行护理,B组给予常规护理.结果 治疗前两组患者的血压(BP)差异无统计学意义(P>0.05);治疗前盐敏感组患者的左室重量指数LVMI,(159.24±18.32)g/m^2]、血清肌酐Cr,(113.25±13.87)μmol/L]、胰岛素抵抗指数HOMA-IR,(3.68±0.82)]、体质量指数BMI为(25.17±4.21)kg/m^2]及尿微量白蛋白MAU为(108.57±21.06)mg/L]等高血压靶器官损害指标均高于非盐敏感组LVMI为(151.07±19.53) g/m^2、Cr为(105.36±12.58) μmol/L、HOMA-IR为(2.98±0.54)、BMI为(23.55±3.86) kg/m^2及MAU(86.54±19.52)mg/L],差异均有统计学意义(均P<0.05);治疗1年后,盐敏感组和非盐敏感组A组的LVMI为(27.36±6.59)g/m^2和(23.51±5.38)g/m2,Cr为(5.82±1.57) μmol/L和(5.46±1.69)μmol/L,BMI为(1.28±0.31)kg/m^2和(1.16±0.29)kg/m^2,MAU(68.25±16.37)mg/L和(65.19±17.92)mg/L等高血压靶器官损害指标以及血压的改善值均大于本组的B组(均P<0.05).结论 盐敏感性高血压患者的靶器官损害较非盐敏感高血压患者更明显;在临床治疗同时,应用针对性护理对策可明显改善患者的血压控制情况及靶器官损害程度.

The efficacy of nursing strategy to the target organ damage of Salt-sensitive hypertension and non-salt-sensitive hypertensive

Abstract:Objective To explore the efficacy of nursing strategy to the target organ damage of Salt sensitive hypertension and non- salt-sensitive hypertensive. Methods 116 patients with hypertension were divided into salt sensitive group and non -sensitive salt group according to the salt stress test results and there were 58 cases in each group. Each of the two groups of patients were divided into A and B two small group and there were 29 cases in each group. The both A groups were given targeted care operations and the both B groups were given routine care operations on the basis of con-ventional antihypertensive therapy. Results There was no significant difference of the Blood pressure (BP) of salt-sensi-tive group and non -sensitive salt group before treatment(P〉0.05). The target organ damage index of the salt-sensitive group such as left ventricular mass index (LVMI) ( 159.24 ± 18.32) g/m^2 , serum creatinine (Cr) ( 113.25± 13.87)μmol / L, insulin resistance index (HOMA-IR) (3.68±0.82), body mass index (BMI) (25.17±4.21) kg/m2 and microalbuminuria (MAU) (108. 574±21.06 ) mg/L were higher than those of non- salt-sensitive group such as hypertension and LVMI(151. 07±19. 53) g/m^2 , Cr(105. 36±12. 58) μmol / L, HOMA-IR(2.98±0.54), BMI (23.55±3.86) kg/ m^2 and MAU (86.54±19.52) mg / L before treatment(P〈0.05) ; The target organ damage index of the both A groups such as LVMI (27.36±6.59) g/m^2 , (23.51±5.38) g/m^2 ,Cr(5. 82±1. 57)μmol/L, (5.46±1.69)μmol / L and BMI (1.28±0.31) kg/m^2 , (1.16±0.29) kg/m^2 and MAU(68. 25±16. 37) mg/L ,(65.19±17.92) mg/L and other hyper tensive target organ damage index , and BP values were greater than the improvement in the both B groups (P〈0.05).
