腹腔镜术中局部注射垂体后叶素和氨甲蝶呤治疗异位妊娠

目的探讨垂体后叶素和氨甲蝶呤用于腹腔镜下治疗输卵管妊娠的临床价值。方法选取未破裂输卵管妊娠患者136例,随机分为观察组68例(腹腔镜下开窗取胚术+局部注射垂体后叶素和氨甲蝶呤),对照组68例(腹腔镜下开窗取胚术),观察2组患者临床指标变化。结果观察组较对照组术中出血量减少,止血时间、手术时间缩短,β-HCG水平术后下降较快,差异均有统计学意义(P<0.05)。观察组持续异位妊娠1例,对照组11例,2组差异有统计学意义(P<0.05)。结论垂体后叶素和氨甲蝶呤用于腹腔镜下治疗异位妊娠可减少术中出血量及预防术后持续异位妊娠。

Treatment of ectopic pregnancy by intraoperative local injection of MTX and pituitrin in laparoscopic conservative surgery

Objective To investigate the clinical effects of pituitrin and MTX in treatment of tubal pregnancy by laparoseopic conservative surgery. Method 136 patients （ from 2004 to 2008 ） with tubal pregnancy （ not ruptured） were randomly divided into two groups, group A （68 patients） with laparoscopic salpingostomy was locally injected pictuitrin and MTX, group B （ 68 patients） was treated with laparoscopic salpingostomy alone. The clinical effects were observed. Results Patients in group A bled less during the operation and serum β-HCG values dropped more quickly after operation than group B. The two groups were statistically different （ P 〈 0.05 ） on these respects. The occurrence of PEP in group A had only 1 and group B had 11 patients, they were statistically different too （ P 〈 0.05）. Conclusion Pituitrin and MTX could be used in laparoscopic conservative surgery to reduce operation bleeding and prevent the occurrence of ectopic pregnancy.
