糖尿病儿童口腔常见致病菌的定量分析及与血糖控制水平的相关性

摘    要:目的 检测糖尿病儿童口腔常见致病菌的含量,探讨其与血糖控制水平的相关性.方法 选取学龄期糖尿病儿童40例和同期体检健康的学龄期儿童50名,收集龈上菌斑,采用DNA聚合酶联反应法(PCR)检测牙龈卟啉单胞菌(Pg)、伴放线放线杆菌(Aa)、中间普氏菌(Pi)及福塞坦氏菌(Tf)4种主要牙周致病菌的相对含量.采用Spearman单因素相关分析4种主要牙周致病菌的相对含量与血糖控制水平的相关性.结果 糖尿病儿童空腹血糖(FPG)、餐后2h血糖(2hPG)及糖化血红蛋白(HbA1c)高于对照组(P<0.05).糖尿病儿童Pg、Pi及Tf相对表达含量高于对照组(P<0.05),2组间Aa含量比较,差异无统计学意义(P>0.05).Pg、Pi及Tf均与FPG、2 hPG和HbA1c呈正相关(P<0.05).Aa与FPG、2 hPG和HbA1c无相关性(P>0.05).结论 糖尿病儿童口腔常见致病菌的相对表达含量升高,其含量与血糖控制水平相关.

Abstract:Objective To examine the quantity of common pathogen in oral cavity of school-ages children with diabetes,and analyze the correlation between pathogen and blood glucose level. Methods Forty school-aged children with diabetes and 40 healthy school-aged controls were selected.The supragingival plaque samples were collected and the relative amount of Porphyromonas gingivalis(Pg), Actinobacillus actinomycetemcomitans(Aa), Prevotella intermedia(Pi) and Tannerella forsythensis(Tf) was detected by polymerase chain reaction(PCR). The correlation between the pathogen and blood glucose level were analyzed. Results The FPG, 2hPG and HbA1c were higher in patients than in the conrols(P<0.05). The amount of Pg,Piand Tf was higher in patients than in the controls(P<0.05) and there was no significant difference in Aa between two groups(P>0.05). Pg,Pi and Tf were positively correlated to FPG,2hPG and HbA1c(P<0.05). Aa had on correlation with FPG, 2h PG and HbA1c(P >0.05). Conclusion The relative amount of common pathogen in oral cavity of school-aged children with diabetes increased and the expression level was associated with blood glucose level.
