颅内血块吸除器之研制及实验研究

摘    要:颅内血块吸除器是一种新的器械。这种器械在吸引器基础上增加一轴心螺旋绞丝(或绞刀),用牙科钻带动可边打碎边吸引颅内块型血肿,一改过去颅内血块型血肿必须开颅摘除的传统方法。而且作者结合运用他们以前创用的有槽手锥锥颅法,不必钻孔,不必切开头皮,锥孔即可完成,十分简便,此法尤适用于高血压脑出血年老体衰不适开颅手术之患者。在体外实验上,明确证明了单纯用吸引器仅能吸住血块但不能将其吸出,而开动绞丝后则极容易且迅速地吸除血块,效果十分明显,我们还建立了兔子颅内血肿实验动物模型(过去尚未见报道)。而且清除动物实验模型的血肿也取得成功。通过实验还确定了绞丝的转速及吸引器负压之最佳参数。

Abstract:In this paper, a new device was described in the use ofdestruction and removal of dense blood clots, and of those of intrace-rebral hematoma. This device is a special instrument with a mandrel-like Archimedesscrew inside a cannula proposed by Kandel El. We have improvedthe design of the instrument and procedures of the operation whichhad been used in the experiments. In the experiments, the hematoma in vitro could be disruptedand aspirated by this instrument. Furthermore, we had establishedan intracerebral hematoma pattern in rabbits. It was also proved thatthe intracerebral hematoma could be disrupted and aspirated. Throughthese experiments we also determined the optimum parameters whichconsist of the speed of screw rotation and negative pressure of asuction.
