骨科围手术期抗菌药物应用的对照研究

目的考查依据《抗菌药物临床应用指导原则》加强管理后骨科围手术期抗菌药物应用情况。方法对2005年3月81例骨科Ⅰ、Ⅱ类切口手术患者和2006年3月84例骨科Ⅰ、Ⅱ类切口手术患者抗菌药物使用情况进行比较调查。结果2005年组、2006年组围手术期预防性应用抗菌药物使用率分别为100％、89．29％（P〈0．05），2006年组手术前期、手术后期抗菌药物使用率下降（P〈0．05），手术期使用率提高（P〈0．05），术后预防用药时间缩短（P〈0．05）。2006年组应用二联、三联抗菌药物者比2005年组减少。结论依据《抗菌药物临床应用指导原则》加强骨科围手术期抗菌药物应用管理行之有效，但仍有不足之处需进一步加强管理。

A Case-control Study on the Application of Antibiotics in Orthopedics during the Perioperative Period

Objective To investigate the antibiotic use during perioperative period in orthopedics after the admission according to Antibiotic Drug Use Guideline.Methods The use of antibiotics among 81 outpatients in Mar.2005 and 84 cases in Mar.2006 during the perioperative period in orthopedics were surveyed.Results The Preventive use rate of antibiotic medication in orthopedics in 2005 and 2006 were respectively 100% and 89.29%(P<0.05);compared with that in 2005 the percentage of preoperative and postoperative antibiotic medication in 2006 was lower(P<0.05);the percentage of intraoperative medication was higher(P<0.05);the duration of postoperative medication was shorter(P<0.05) and the percentage of 2,3 or 4 kinds of antibiotics combined treatment was lower(P<0.05).Conclusions The administration of the medication of antibiotics during perioperative period has been proven effective,but there are some problems needed to be managed further.
