腹腔镜经腹腹膜前补片植入术的临床应用

目的：探讨腹腔镜经腹腹膜前补片植入术（ TAPP ）治疗腹股沟疝的临床疗效和安全性。方法：选取2010年4月至2013年2月我院收治的86例腹股沟疝患者，随机平均分为两组，对照组和实验组，用传统的无张力疝修补术治疗对照组患者，用腹腔镜经腹腹膜前补片植入术治疗实验组患者，观察两组患者手术时间，术后1d疼痛评分，术后对患者随访3个月，记录两组患者术后住院时间、并发症发生率。结果：实验组手术时间、住院时间和术后1d疼痛评分均优于对照组，t 值分别为12．19、9．17、13．60（P＜0．05），差异有统计学意义；实验组发生并发症例数为3例，发生率6．98％，明显低于对照组发生并发症例数为9例，发生率20．93％，X^2＝4．49，P＜0．05，差异有统计学意义。结论：腹腔镜经腹腹膜前补片植入术治疗腹股沟疝不仅具有创伤小、疼痛轻、恢复快、复发率低等优势，而且其最大限度地缩小和隐蔽手术疤痕，达到美容效果，在临床手术中有十分大的应用价值。

The Clinical Application of Transabdominal Approach with Stapled Pre-peritonealmesh Repair

Objective：To observe clinical efficacy and safety of the transabdominal approach with stapled pre-peritonealmesh repair .Method： 86 cases with inguinal hernia patients in our hospital from April＆amp;nbsp;2010 to February 2013 were randomly divided into two groups： control group and experimental group .The control group was treated with traditional hernia repair treatment , and the experimental group was treated with transabdominal approach with stapled pre-peritonealmesh repair , the two groups were observed operative time, postoperative pain scores one day after operation , patients were followed up for three months , recorded two groups of postoperative hospitalization time , complications rate .Result： The experimental group was much better than the control group on the operative time , length of hospital stay and postoperative one day after operation pain scores, The value of test was respectively was 12.19,9.17, 13.60（ P〈0.05）,the difference has statistically significant; complications occurred in the experimental group was 3 cases, the complication rate was 6.98%, It was significantly lower than the control group , the number of cases of com-plications occurred in 9 cases, the complication rate was 20.93%, X^2=4.49, P 〈0.05, the difference has statistically significant .Conlcusion： The application of transabdominal approach with stapped pre -peritonealmesh repair in inguinal hernia is not only less invasive , less pain ,faster recovery , lower relapse rate , as well as its maximum hidden scar reduction and achieve cosmetic results , it has large applications in clinical surgery.
