上海市奉贤区农村中老年人骨关节炎流行病学调查

目的了解奉贤区农村中老年人骨关节炎(osteoarthritis,OA)的流行病学特征,为OA的病因探索和防治研究提供依据。方法从上海市奉贤区东、西、南、北、中5个方向进行整群随机抽样,抽取5个社区,并从5个社区各抽取一个村,每村抽3个村民小组。再根据奉贤区公安局相关派出所的户籍资料,从上述15个村民小组户籍资料中抽取1965年6月1日0时以前出生的897名中老年人为调查对象。在获得受检者知情同意后进行流行病学调查和腰椎、颈椎、手及膝关节放射学检查。调查内容包括一般人口学资料及职业史、现病史、既往史、关节症状、关节功能等。结果中老年人OA总患病率为58.19%,男女患病率分别为48.81%和65.00%,差异有统计学意义(P<0.01)。腰椎、颈椎、手关节、膝关节的OA患病率男女间比较,差异均有统计学意义(P<0.01)。中老年人临床OA患病率随年龄增长而增高(P<0.01)。结论奉贤地区农村中老年人OA患病率随年龄增长而增高,腰椎、颈椎、手关节和膝关节的OA患病率女性均高于男性。

Epidemiological Investigation of Otsteoarthritis in Rural Middle-aged and Senior Residents in Fengxian District of Shanghai

Objective To investigate the epidemiological features of osteoarthritis(OA)in rural middle-aged and senior residents in Fengxian district of Shanghai in order to provide evidences for study on pathogenesis and prevention and treatment of OA.Methods A village in each of the 5 communities was drawn out from five directions of the east,south,west,north,and middle parts of Fengxian district by cluster randomized sampling,and then three groups were sampled from each village.Totally 897 people born before zero o'clock sharp,1st June,1965 were inquired according to their residential files in Fengxian police station.After writing informed consent,the epidemiological features and radiological examination of cervical vertebrae,lumber vertebrae,hands and knee joints were surveyed.The contents of investigation included general census,occupations,history of present illness,history of post illness,symptoms of joints and function of joints.Results The total incidence of OA in the middle-aged and senior residents was 58.19%.And male incidence and female incidence were 48.81% and 65.00% respectively,with a significant difference between male and female(P<0.01).In the OA incidences of lumbar vertebrae,cervical vertebrae,hand joints and knee joints there were significant difference between male and female(P<0.01).The incidence of OA was increased as the age got increased(P<0.01).Conclusion The total OA incidence in rural middle-aged and senior residents in Fengxian district was increased as the age get increased.The OA incidences of lumbar vertebrae,cervical vertebrae,hand joints and knee joints in female are higher than those in male.
