新型临时血管转流装置在犬股动脉损伤再通的应用

摘    要:目的本研究自行设计新型血管转流装置,并探讨其在犬股动脉损伤中的应用价值。方法取健康成年比格犬12只,体重(20±2)kg,通过手术制作股动脉损伤动物模型,使用新型血管临时转流装置联通离断的股动脉,按随机数表法分为A组、B组、C组,每组各4只。各组均于植入血管转流装置,术后每2 h进行静脉采血,行血常规、凝血常规检查;A组植入术后4 h处死,B组6 h处死,C组8 h处死,切取装置两侧股动脉进行组织学切片检查。结果血常规及凝血常规检查:白细胞计数(WBC)、血红蛋白(HGB)、血小板计数(PLT)、凝血酶原时间(PT)、凝血酶时间(TT)、部分活化凝血酶时间(APTT)、Fib(纤维蛋白原)各项指标术前、处死前比较差异无统计学意义(>0.05);术前,植入术后4 h、6 h、8 h通过组织学检查未见动脉明显损伤。结论该新型血管临时血管转流装置自带固定球囊,可减少血管损伤,在短时间内可有效临时联通离断的股动脉,恢复下肢血液灌注。

Abstract:Objective To design a new type of temporary vascular shunt device and evaluate its application in canine femoral artery injury. Methods A total of 12 healthy adult beagle dogs weighing (20±2) kg were operated on to establish an animal model of femoral artery injury. The severed femoral artery was connected by the temporary vascular shunt device. The dogs were divided into group A, group B and group C by random number table, with 4 in each group. Each group received blood routine and coagulation routine examination every 2 hours after implantation; group A was executed 4 hours after implantation, group B was executed 6 hours, group C was executed 8 hours, and bilateral femoral arteries of the device were taken for histological examination. Results There was no significant difference in WBC, HGB, PLT, PT, TT, APTT and Fib before and after death (P>0.05). Histological examination showed no significant damage to the arteries pre-op, postoperative 4h, 6h, 8h. Conclusion The new temporary vascular shunt device has a fixed balloon, which reduces vascular injury, can effectively connect the severed femoral artery and restore the blood perfusion of lower limbs in a short time.
