经椎弓根"中华长城"手术治疗胸腰椎爆裂骨折

为进一步评价后路椎弓根螺钉内固定治疗胸腰椎爆裂骨折的治疗效果。将47例胸腰椎爆裂骨折按内固定方式分为治疗组22例、对照组25例，分别采用中华长城短节段椎弓根螺钉内固定和AF内固定。观察术前、术后X线片椎体前、后缘高度厦Cobb角变化情况。结果显示两组椎体前后缘高度和Cobb角术前、术后差异均有非常显著性意义（P〈0．01）；术后Cobb角恢复治疗组明显好于对照组，差异有非常显著意义（P〈0．01）。表明中华长城短节段椎弓根螺钉系统内固定治疗胸腰椎爆裂骨折效果满意，尤其是矫正骨折畸形，恢复脊柱生理弧度具有明显效果。

THE TREATMENT OF THORACOLUMBAR- VERTEBRAL BURST FRACTURE BY TRANS- VERTEBRAL- ARCH- PEDICLE "CHINESE GREAT WALL" SURGERY

In order to further evaluate the therapeutic effect of posterior vertebral-arch-pedicle-screw internal fixation on thoracolumbar-vertebral burst fracture(TLVBF),47 cases of TLVBF were divided into two groups: the treated(22 cases) and the control(25 cases),which were treated by short-segmental vertebral-arch-pedicle-screw internal fixation of "Chinese Great Wall"(SVIF) and by AF internal fixation,respectively;the changes of the Cobb's angles and the heights between the anterior and posterior vertebral-body edges on the pre-and post-surgical X-rays were observed.The results showed that the pre-and post-surgical Cobb's angles and the heights between the anterior and posterior vertebral-body edges had a very significant difference(P < 0.01);the post-surgical Cobb's angle recovery in the treated group was significantly better than that in the control with a very significant difference(P < 0.01),suggesting that the treatment of TLVBF by SVIF has a satisfactory result,especially in the correction of the fracture deformity and the recovery of the spine physiological curvature.
