高强度间歇训练对中老年2型糖尿病患者干预效果的meta分析

摘    要:目的 系统评价高强度间歇训练对中老年2型糖尿病患者心脏代谢危险因素与BMI的影响。方法 计算机检索EBSCOhost等数据库,收集相关文献,使用Stata 12.0软件进行统计学处理。结果 共纳入文献11篇,其中5篇为A级,6篇为B级,研究间无发表偏倚。Meta分析显示:高强度间歇干预后明显改善患者糖化血红蛋白(WMD=-0.62,95%CI:-0.81~-0.43)、空腹血糖(SMD=-0.87,95%CI:-1.25~-0.49)、2-h后空腹血糖(WMD=-2.17,95%CI:-2.69~-1.65)、胰岛素(SMD=-0.93,95%CI:-1.42~-0.44)、胰岛素抵抗(SMD=-0.82,95%CI:-1.24~-0.40)、高密度脂蛋白(SMD=0.75,95%CI:0.33~1.18)含量;同时降低患者血压水平(收缩压WMD=-5.20,95%CI:-9.45~-0.94;舒张压WMD=-3.45,95%CI:-6.17~-0.73)和BMI(WMD=-1.34,95%CI:-2.21~-0.48)。对胆固醇、低密度脂蛋白和甘油三酯均无明显影响。结论 高强度间歇训练能改善中老年2型糖尿病患者血糖、血脂的代谢能力和BMI。建议广大中老年2型糖尿病患者采用高强度间歇训练。

Meta-analysis of the intervention effect of high intensity interval training on type 2 diabetes in middle-aged and elderly patients

Abstract:To systematically evaluate the effects of high-intensity interval training on cardiometabolic risk factors and BMI in middle-aged and elderly patients with type 2 diabetes. Methods EBSCOhost, Web of Science and other databases were searched by computer, relevant literatures were collected, and Stata 12.0 software was used for statistical processing. Results A total of 11 references were included, of which 5 were grade A and 6 were grade B. There was no inter-study publication bias.Meta-analysis showed that compared with the control group, patients with high-intensity intermittent intervention were significantly improved in hemoglobin a1c (WMD=-0.62, 95%CI: -0.81--0.43), fasting blood glucose (SMD=-0.87, 95%CI: -1.25--0.49), 2-h fasting blood glucose (WMD=-2.17, 95%CI: -2.69--1.65), insulin (SMD=-0.93, 95%CI: -1.42--0.44), insulin resistance (SMD=-0.82, 95%CI:-1.24--0.40) and high density lipoprotein (SMD=0.75, 95%CI: 0.33-1.18).At the same time, the patient’s blood pressure was reduced (systolic blood pressure WMD=-5.20, 95%CI: -9.45--0.94; diastolic blood pressure WMD=-3.45, 95%CI: -6.17--0.73) and BMI (WMD=-1.34, 95%CI: -2.21--0.48).There was no significant effect on cholesterol, low density lipoprotein and triglyceride. Conclusion HIIT can improve blood glucose, lipid metabolism and BMI in middle-aged and elderly patients with type 2 diabetes. It is suggested that the middle-aged and elderly patients with type 2 diabetes should adopt HIIT.
