两种手术方式对非小细胞肺癌手术对照研究

目的比较胸腔镜与开胸手术治疗非小细胞肺癌根治性及围术期恢复情况。方法分析手术治疗的非小细胞肺癌患者。按手术方式分为胸腔镜组和开胸组各46例。比较两组围术期血清应激激素、白细胞及术后应激反应。比较两组手术相关、围术期各项指标的差异。结果两组患者术前血WBC、血清应激激素水平无差异。胸腔镜组术后血清Cor、血白细胞及CRP水平较低,胸腔镜组手术时间长、术后住院时间短,胸腔镜组术后引流量少。两组手术指标及术后并发症无差别。结论在安全性及根治性一致的情况下,与开胸相比,胸腔镜具有术后应激反应轻、痛苦小、恢复快等优势,可作为非小细胞肺癌首选治疗方式。

Comparison of two methods of operation on pulmonary lobotomy in the treatment of patients with non-small cell lung cancer at stage

Objective To compare the surgical trauma and perioperative recovery between VATS and thora cotomy for lobotomy. Methods In accordance with the operative methods, patients were separated into two groups： the VATS group （46 patients） and the open chest group （46 patients）. The level of white blood cell, serum Cortisol （Cor） and C-reactive protein （CRP） were tested from perioperative non-small cell lung cancer patients. Many surgi- cal indexes were chosen to measure operation result and perioperative recovery. Results There was no statistical significance in preoperative level of serum Cor, WBC and CRP. The postoperative concentrations of serum Cor, WBC, CRP were lower in the VATS group than in the open chest group. And there was obviously statistical significance in duration of operation and postoperative hospital stay. There was no statistical significance in surgical indexes and postoperative complications. Conclusion VATS lobotomy is able to achieve the same result of radical treatment out-come and safety as open chest lobotomy in patients. Compared with routine open chest lobotomy, VATS lobotomy has less surgical trauma, faster recovery, which can be recommended for patients with non-small cell lung cancer.
