宫颈癌根治术后泌尿系感染细菌谱及敏感性分析

摘    要:目的了解宫颈癌根治术后泌尿系感染情况、感染细菌谱和药物敏感性。方法对我院1998年8月—2008年4月期间收治的214例行宫颈癌根治术的患者的尿液细菌培养资料进行回顾性分析。结果在214例宫颈癌根治术患者中,术后发生泌尿系感染者67例,发生率为31.31%。共分离出病原菌88株,18例为2种或2种以上病原菌混合感染,其中革兰氏阴性菌38株,占43.18%;革兰氏阳性菌34株,占38.64%;真菌及支原体16株,占18.18%。感染的病原菌以粪肠球菌、大肠埃希菌及真菌为主,分别占22.73%、21.57%和15.91%。革兰氏阴性菌对丁胺卡那敏感率较高,均达80.00%以上,革兰氏阳性菌对万古霉素敏感,敏感率为89.47%。结论掌握宫颈癌根治术后泌尿系感染细菌谱及药物敏感情况,有针对性地合理使用敏感抗生素,有助于有效预防和控制术后泌尿系感染。

Bacterial Species Distribution and Antibiotic Susceptibility in Patients with Post Radical Hysterectomy Urinary Tract Infection

Abstract:OBJECTIVE Investigate the state of urinary tract infection in post radical hysterectomy patients, and study its bacterial species distribution and antibiotics susceptibility. METHODS The medical records of 214 cases that had undergone radical hysterectomy in our hospital from August 1998 to April 2008 were retrospectively reviewed. RESULTS Of the 214 cases, 31.31% (67 cases) got urinary tract infection(UTI). 88 strains were isolated 43.18% gram-negative isolates, 38.64% gram-positive isolates, and 18.18% fungi and mycoplasmas. The most frequently isolated bacterial species were enterococcus faecalis(22.73%), escherichia coli(21.57%), and fungi (15.91%). Gram-negativeisolates were more sensitive to amikacin, while gram-positive isolates were highly sensitive to vancomycin. And the susceptibilities were over 80.00% and 89.47% respectively. CONCLUSION Grasping bacterial species distribution and antibiotics susceptibility of post radical hysterectomy urinary tract infection can direct rational clinical use of antibiotic and will be helpful to prevent and reduce postoperative urinary tract infection.
