心脏重症监护病房患者分级药学监护档案表的建立

目的：建立心脏重症监护病房（CCU）个体化分级药学监护档案表，对患者进行全程药学监护。方法：结合CCU临床诊疗实际情况，回顾性搜集、整理和掌握患者的用药信息，根据实际情况定义监护等级，创建分级药学监护档案表。结果：分级药学监护档案表的制定大大提高临床药师的工作效率，患者的用药情况得到极大的规范和合理。结论：建立分级药学监护档案表，将协助医师用药做到个体化，努力实现“用药者健康利益最大化，药源性损害与成本最小化”。

Establishment of the grading pharmaceutical care ifle table for CCU patients

Objective: To establish a grading pharmaceutical care ifle table for CCU patients so that the whole process of pharmaceutical care can be implemented to the patients.Methods:Medication information about the patients was collected and retrospectively analyzed based on the actual situation of the clinical diagnosis and treatment at CCU, the monitoring level was deifned based on the actual situation and a grading pharmaceutical care ifle table was ifnally established.Results: The working efifciency of the clinical pharmacists could be greatly improved and medication in the patients could be greatly standardized and trended to be more reasonable by the established table.Conclusion: The table will help physicians achieve individualized medication, and make an effort for drug users to achieve maximal health beneifts and minimal drug-induced damage and cost.
