膀胱灌注化疗致局灶性膀胱炎(附26例报告)

目的 :探索膀胱灌注化疗所致的特殊性膀胱炎的诊断和治疗方法。方法 :回顾分析 1988年 5月～ 2 0 0 1年 8月对保留膀胱的膀胱癌术后病人 ,采用膀胱灌注化疗后经膀胱镜和病理证实并发局灶性膀胱炎临床资料。结果 :137例术后定期膀胱灌注化疗 ,2 6例发生了局灶性膀胱炎 ,发生率为 18.9% ,2 1例作了活检 ,3例误诊癌复发而行手术治疗 ,其余均对症药物治愈。结论 :膀胱灌注化疗所致的膀胱炎通常为局灶性 ,膀胱镜下所见炎症类型多样 ,有时可误诊为癌复发 ,诊断主要依靠膀胱镜及病理检查 ,治疗单用抗生素效差 ,合用糖皮质激素效佳。

LOCALIZED CYSTITIS CAUSED BY THE INTRAVESICAL INSTILLATION CHEMOTHERAPY

Objective:To explore the diagnosis and treatment of the special cystitis resulted from the intravesical instillation chemotheraphy. Method:The clinical data of localized cystitis caused by intravesical instillation chemotherapy in patients with bladder cancer after partial cystotomy, which was confirmed by cystoscope and biopsy, were retrospectively analyzed from May 1988 to August 2001. Results:Localized cystitis occurred in 26( 18.9%),biopsy was perfomed in 21 cases, 3 patients underwent open operation because of wrong diagnosis and the other patients took drugs. Conclusions: The type of peculiar cystitis was usually localized and varied in cystoscope examination.Antibiotic combined with cortical hormones is an effective treatment.
