应变率成像技术对系统性红斑狼疮患者左室局部心肌收缩及舒张功能评价

目的:应用应变率成像技术评价系统性红斑狼疮(SLE)患者左室局部心肌收缩及舒张功能,以提示心肌早期功能受损可能,为临床诊治提供参考.方法:SLE患者68例(SLE组)与正常对照组55例,采集左室基底段和中间段心尖四腔、三腔、二腔观各室壁的应变率曲线,测量并比较两组收缩期、舒张早期、舒张晚期峰值应变率SRs、SRe、SRa,计算SRe/SRa,测量并比较两组左室射血分数(EF)、短轴缩短率(FS)和二尖瓣口血流的E峰(VE)、A峰(VA)、计算VE/VA.结果:左心室收缩功能各参数比较,SLE组与正常对照组之间EF、FS比较差异无统计学意义(P＞0.05),SRs低于正常对照组(P＜0.05);左心室舒张功能各参数比较,SLE组与正常对照组之间VE、VA、VE/VA、Sra比较差异无统计学意义(P＞0.05),而SRe、SRe/SRa低于正常对照组(P＜0.05).结论:应变率成像技术相对不受心脏整体运动和局部牵拉的影响,能够真实反映局部心肌内在力学特征;SRs、SRe、SRe/SRa可以作为评价左室局部收缩和舒张功能的有效指标.

Assessment of Left Ventricular Regional Myocardial Systolic and Diastol Function in Patients with Systemic Lupus Erythematosus by Strain Rate Imaging

Objective： To evaluate left ventricular regional myocardial systolic and diastol function in patients with systemic lupus erythematosus by strain rate imaging in order to offer reference for clinical treatment. Methods ：68 SLE patients and 55 healthy volunteers were enrolled, strain rates were measured with SRI at four - chamber, three - chamber and two - chamber views, SRs, SRe, SRa, SRe/SRa were calculated and compared. EF, FS, VE, VA,VE/VA were detected. Results：Compared with the normal control group, SRs, SRe, SRe/SRa were depressed in SLE group （ P 〈 0.05 ） , while there were no significant differences of EF, FS, VE,VA, VE/VA, SRa between the two groups （ P 〉 0. 05 ）. Conclusions： SRs, SRe, SRe/SRa can be used as effective indicators of left ventricular regional systolic and diastolic function.
