山东省肺结核患者间接经济负担影响因素分析

目的了解山东省肺结核患者的间接经济负担,分析其影响因素,为进一步完善中国结核病防制工作提供科学依据。方法采用分层整群随机抽样的方法,抽取山东省6县(区)36个乡镇2006年10月1日—2007年9月30日登记的已结束治疗的肺结核涂阳患者进行面对面的问卷调查。结果山东省501例已经结束治疗的肺结核患者平均误工时间为27.21 d,例均间接经济负担为193.97元,中位数为73.17元;相关分析结果表明,确诊延误时间、确诊前就诊次数均与肺结核患者间接经济负担呈正相关(P<0.001);多因素logistic回归分析结果表明,肺结核患者间接经济负担的主要影响因素是确诊前就诊次数、是否住院、地区、初次就诊单位。结论肺结核患者间接经济负担较高,进一步采取措施减少患者及其家属在治疗过程中的误工时间,可缓解患者的间接经济负担。

Analysis on influence factors of indirect economic burden of tuberculosis patients in Shandong province

ObjectiveTo explore the indirect economic burden and its influence factors among tuberculosis(TB) patients in Shandong province and to provide scientific basis for TB prevention and control in China.MethodsA faceto-face questionnaire survey was conducted among registered smear-positive TB patients selected with stratified random cluster sampling from 36 towns of 6 counties or districts from October 1,2006 to September 30,2007.ResultsAmong the 501 TB patients at the end of treatment,the average indirect economic burden of one patient was 193.73 RMB yuan, with a median of 73.17 yuan.The influence factors of indirect economic burden of the patients were time of visiting a doctor before confirmatory diagnosis of TB,hospitalization,residential place,and the first medical institution for the diagnosis.ConclusionIndirect economic burden of TB patients is high and measures should be taken to reduce indirect economic burden of TB patients in Shandong province.
