胸水中CEA CYFRA21-1 NSE和LDH对肺癌的临床诊断

摘    要:目的:探讨CEA、CYFRA21-1、NSE、LDH水平对癌性胸水的诊断价值。方法:采用电化学发光方法检测胸水中的CEA、CYFRA21-1和NSE,采用酶法检测LDH。结果:恶性胸水的CEA、CYFRA21-1、NSE、LDH水平明显高于良性胸水(P<0.01),肺腺癌CEA明显高于鳞癌和小细胞肺癌(P<0.01),肺鳞癌CYFRA21-1水平明显高于腺癌(P<0.05)和小细胞肺癌(P<0.01),小细胞肺癌NSE明显高于鳞癌(P<0.01)。4项标志物联合检测的敏感性、准确性均比单项检测高。结论:联合检测胸水中的CEA、CYFRA21-1、NSE和LDH对肺癌的诊断具有重要的临床意义。

Abstract:Objective:To e xplore the value of detecting CEA?CYFRA21-1?NSE?LDH in the pleural effusi on in the malignant pleura effusion diagnosis.Methods:The levels of CEA?CYFR A21-1?NSE in the pleural effusion were assayed by electrochemiluminescence immunoassay.The levels of LDH in the pleural effusion was assayed by enzyme me thod.Results:The CEA?CYFRA21-1?NSE?LDH levels were notably elevated in malignant pleura effusion compared with benign pleura effusion (P<0.01) .The level of CEA in adenocarcinoma was notably elevated com-pared with that of squamous cell carcinoma and small cell lung cancer(P<0.01);The level of CYFRA21-1in squamous cell carcinoma was notably elevated compared with that of adenocarci-noma (P<0.05)and small cell lung cancer(P<0.01 );The NSE level in small cell lung cancer was notably elevated compared with t hat of squamous cell carcinoma (P<0.01).The sensitivity and ac-curacy o f the combined detection of CEA?CYFRA21-1?NSE?LDH were high compared with those of single-serological index.Conclusion:The combined detection of CEA? CYFRA21-1?NSE and LDH in the pleura effusion were of important clinical si gnificant in the diagnosis of lung cancer.
