532例老年心力衰竭患者发病诱因特点分析

目的探讨老年心力衰竭（HF）的发病诱因,为提高老年HF防治水平提供临床依据。方法对长海医院近5年老年HF患者532例进行回顾性总结分析。结果老年HF患者发病季节特点：冬季占全年41.5%,春季占21.4%,夏季11.5%,秋季25.6%;老年人HF临床主要诱因为感染138例（26.0%）、心肌缺血127例（23.8%）、劳累99例（18.7%）、心律失常28例（5.2%）、其他如情绪激动43例（8.0%）,输液过快25例（4.7%）,不适当药物使用21例（4.0%）也是老年HF常见原因之一。结论老年HF发病时间存在明显的季节差异,表现为冬高夏低,在老年人群尤其突出。老年人HF临床主要诱因为感染、心肌缺血、心律失常。

Inducing factors of heart failure in the elderly patients: analysis of 532 cases

Objective To analyze the factors inducing acute attack in old adults with heart failure(HF). Methods The data of 532 elderly patients who had diagnoses of HF in this hospital were studied retrospectively using the EXCEL HF DataBank.The inducing factors of HF were analyzed. Results Season regularity of attack in old adults with HF:winter accounted for 41.5% of the whole year,spring accounted for 21.4%, summer 11.5%, and autumn 25.6%.Infection and myocardial ischemia were the most common inducing factors. The third common factor was tiredness and the other factors were emotional agitation, arrhythmia, fast transfusion, improper treatment, etc. Conclusion Infection was the main factor inducing acute attack in old adults with HF. Season difference is also a factor influencing the attack of heart failure. Attention should be paid to these characteristics in old adults with HF during the diagnosis, treatment and prevention for HF.
