青光眼不同视盘表现的彩色多普勒超声研究

摘    要:目的:观察不同视盘表现的青光眼患者球后血管的血流速度并与正常视盘者进行对比,方法:经视盘照相的青光眼患者中挑选视盘明显属于局部缺血型FI,近视的青光眼视盘MG,老年硬化性视盘SS及视乳头普遍扩大型GE四型者进行彩色多普勒血流显像检查(CDI),测量球后动脉流速及RI值。结果:68只眼不同视盘表现的青光眼患者与24只眼正常视盘者比较,FI组、MG组和GE组视网膜中央动脉舒张末血流速度减低,RI值升高,四组睫状后短动脉舒张末流速减低,RI值升高,SS组GE组眼动脉收缩期血流速度降低。不同视盘比较,GE组视网膜中央动脉、睫状后短动脉舒张末血流速度降低,睫状后短动脉(颞侧,平均的)RI值升高。结论:四组青光眼患者视网膜中央动脉、睫状的短动脉与正常组比较,血流速度降低,RI值升高,具有统计学意义,不同类型视盘表现的青光眼患者中GE组球后血管的CDI显示较大的血流异常。这些血流异常与他们的球后血管阻力升高相关,与青光眼病人不同发病机制有关。

Abstract:Purpose To observe the blood flow velocity in the retrobulbar vesseis of patients with glaucoma with different appearances of optic discs and comparisons normal optic discs. Methods Patients with four different discs appearances (focal ischemic discs, myopic glaucomatous discs, senile sclerotic discs, and discs with a generalized enlargement of the cup) were selected from a pool of optic disc photographs. Color Doppler Imaging (CDI) was performed in those patients whose discs clearly belonged to one of the disc patients. The peak systolic and end diastolic velocity were measured and the resistance index(RI) calculated from the retrobulbar arterys. Results CDI was performed in 68 eyes of patients with glaucoma with four different discs appearances and 24 eyes with normal optic discs. Compared the normal group and the patients with FI group, MG group and GE group had lower diastolic velocity and a higher RI in their central retinal artery(CRA). The patients with four groups had lower diastolic velocity and a higher RI in their short posterior ciliary artery(SPCA). SS group and GE group had lower systolic velocity in their ophthalmic artery. Compared the patients with various optic discs. GE group had lower diastolic velocity in their CRA,SPCA and had a higher RI in their SPCA (TSPCA, MSPCA). Conclusion Compared the normal group with the patients with four groups had statistically significantly lower velocity and higher RI in their CRA,SPCA. Compared the patients with glaucoma with different appearances of optic discs GE group to have greater circulatory abnormalities in their retrobulbar vessels studied with CDI that are compatible with a higher downstream resistance in these vesseis. These circulatory abnormalities could be related to different the pathogenesis of the glaucoma in these patients.
