胃癌合并糖尿病患者围手术期的处理

目的探讨胃癌合并糖尿病患者围手术期处理方法。方法对2007年1月至2008年12月收治的80例胃癌合并糖尿病的患者围手术期处理进行回顾性分析，将40例术后微量泵注射胰岛素者定为研究组，将40例术后胰岛素皮下注射者定为对照组。检测围手术期血糖水平、住院时间及术后并发症。结果研究组和对照组的术前、术中血糖均无显著性差异。研究组和对照组的血糖达标时间、胰岛素使用量、住院时间明显优于对照组（P〈0．05），研究组术后并发症的发生率明显低于对照组（P〈0．05）。结论胃癌合并糖尿病患者术后使用微量泵控制血糖的方法要优于皮下注射胰岛素的方法，可以减少术后并发症的发生。

Perioperative management of the patients with gastric carcinoma and diabetes mellitus

Objective To study the perioperative management of the patients with gastric carcinoma and diabetes mellitus. Methods A total of 80 cases of gastric carcinoma patients with diabetes mellitus were analyzed retrospectively from January 2007 to December 2008, and we divided them into tow groups ：40 cases in the group of study who were used micro-insulin pump, and other 40 cases in the group of control who were used subcutaneous injection of insulin. All patients were observed the glucose level, the target time, the volume of insulin, time of in-hospital and the complications after operation. Results The glucose level in two groups were not significantly different before and during operation. The time of blood glucose for achieving the target in group of study was shorter than that in group of control, and the volume of insulin used in group of study was less than that in group of control, and the time of stay in-hospital in group of study was also shorter than that in group of control（P 〈 0. 05 ）. The incidence rate of complications in group of study were lower than that in group of control （ P 〈 0.05）. Conclusion The method of using micro-pump to control blood sugar is better than the method of subcutaneous injection of insulin, and that can reduce the incidence of complications after operation.
