糖尿病合并高血压的临床特征及其中医证候

目的:分析糖尿病合并高血压患者的临床特征及其中医证候,为糖尿病合并高血压的中医治疗提供依据。方法:以问卷调查的方式收集并分析200例2型糖尿病合并高血压患者的临床特征及其中医证候。结果:2型糖尿病合并高血压患者的临床症状以夜尿增多、口干多饮、目糊、乏力等较为多见。在气血津液辨证中以气虚证为多见,在脏腑辨证中以肾精亏虚证多见;肝、脾、肾受损与本病密切相关,兼痰浊瘀血及肝肾阴亏、心肾不交者多见昼夜节律异常。血压增高与糖尿病并发症的数目、血糖控制不达标以及血脂代谢异常等显著相关(P<0.05)。结论:糖尿病合并高血压的中医证候以虚证为主,病及肝脾肾三脏;血压增高与糖尿病的并发症及血糖控制情况相关。

Clinical Characteristics and Traditional Chinese Medicine Syndromes of Patients with Diabetes and Hyperten- sion

Objective：To investigate the clinical characteristics and traditional Chinese medicine （TCM） syndromes of patients with diabetes and hypertension. Methods：The clinical characteristics and TCM syndromes of 200 patients with type 2 diabetes and hypertension were studied. Results： The most frequent symptoms were nocturia, thirst, polydipsia, blurred vision and fa- tigue. In syndrome differentiation of Qi, blood and body fluid, Qi deficiency syndromes were more common. In syndrome dif- ferentiation of viscera, syndromes of kidney essence insufficiency were more common. The impairment of liver, spleen and kid- ney was closely related to this disease. Abnormal circadian rhythm mostly occured in patients with phlegm and blood stasis, liv- er and kidney yin deficiency and disharmony between heart and kidney. Hypertension was related lo the number of complica- tions of diabetes, unsatisfying blood glucose level and abnormal lipid metabolism（P〈0.05）. Conclusions： Deficiency syndromes are more common in the TCM syndromes of patients with diabetes and hypertension. The impairment of liver, spleen and kid- ney is closely related to this disease. Hypertension is related to the complications of diabetes and unsatisfying blood glucose level.
