Claudin-1、SMA和P63在乳腺上皮增生及乳腺癌组织中的表达及其诊断价值

目的：探讨Claudian-1、SMA和P63联合检测在乳腺癌发生发展中的作用及其诊断价值。方法：用免疫组化SP法检测11例普通型导管增生（UDH）、13例非典型性导管增生（ADH）、10例乳腺导管原位癌（DCIS）及10例浸润性导管癌（IDC）组织中Claudin-1、SMA和P63的表达情况。结果：Claudin-1主要表达于乳腺上皮细胞，表达模式为细胞膜／浆型；与UDC及ADH组相比，Claudin-1在DCIS及IDC组中的表达降低或消失（P〈0．01）；与UDC，ADH及DCIS组相比，IDC组SMA和P63均无表达（P〈0．01）。结论：在普通型导管增生向非典型导管增生到乳腺癌的发生发展过程中，Claudin-1的表达逐渐减弱并消失；Claudin-1联合SMA和P63检测可用于乳腺良性导管上皮增生与乳腺癌的鉴别，Claudin-1可作为乳腺癌诊断的一种标记物。

Expressions of Claudin-1, SMA and P63 in benign breast hyperplasia and breast cancer tissues and their diagnostic value

Objective ：To study the effect of Claudin - 1, SMA and P63 in the pathogenesis of breast cancer and their diagnostic value. Methods： Immunohistochemistry method was used to detect the expressions of Claudin - 1, SMA and P63 in 11 cases of usual ductal hyperplasia （UDH）, 13 cases of atypical ductal hyperplasia （ADH）, 10 cases of ductal carcinoma in situ （ DCIS）, and 10 cases of invasive ductal carcinoma （ IDC ）. Results： Claudin - 1 was localized on the cellular membrane or in cellular plasma of breast epithelium. Compared with UDH and ADH groups, Claudin- 1 expression was down -regulated in DCIS and IDC groups （P 〈0.01 ）. Compared with UDH, ADH and DCIS groups, both SMA and P63 were not detected in IDC group （ P 〈 0.01 ）. Conclusion： The expression of Claudin - 1 in the pathogenesis of breast cancer originated from UDH to ADH is down - regulated gradually and lost finally in the IDC tissues. Combined detection of Claudin - 1, SMA and P63 may be helpful for differential diagnosis of benign breast hyperplasia and breast cancer. Claudin - 1 could be regarded as one of tumor markers of breast cancer.
