CT引导肾囊肿定位穿刺技术的改良与临床应用

目的 提高CT引导下肾囊肿定位穿刺技术水平及临床应用价值.方法回顾分析在CT引导下肾囊肿定位穿刺硬化治疗的患者182例：其中利用改进后定位穿刺技术方法硬化治疗的肾囊肿126例,定位方法是患者平稳呼吸下CT薄层扫描,确定最佳层面和皮肤穿刺点,对外突型肾囊肿的穿刺路径是穿刺针平行肾实质进入囊肿内;利用改良前定位穿刺技术方法硬化治疗的肾囊肿56例,定位方法是患者闭气下CT薄层扫描,确定最佳层面和皮肤穿刺点,对外突型肾囊肿的穿刺路径是穿刺针垂直进入囊肿内;硬化剂均为99.7%的无水乙醇,抽净囊液,注入无水乙醇保留5～10 min抽净.结果改良后治疗的肾囊肿126例均穿刺成功,治愈率91%,有效率100%,改良前56例穿刺成功率89.3%,治愈率89%,有效率100%.结论改良后的CT引导肾囊肿定位穿刺硬化肾囊肿,定位准确,穿刺成功率明显提高,保证治疗效果,值得临床推广应用.

Improved technology of treating renal cyst through the puncture sclerosing therapy under CT guidance

Objective To evaluate the clinical value, of CT - guided percutaneous sclerotherapy of renal cyst aspira- tion. Methods 182 cases with renal cysts who were treated by CT - guided percutaneous aspiration sclerotherapy were analysed. 126 cases of renal cyst were treated by improved technology hardening treatment method. Positioning method is scaning the thin layer through CT under smooth breath, determine the best level and point of piercing, the piercing path for parallel cysts is Paralleling into cysts. 56 cases were treated by the forward positioning impale technology, Positioning method is scaning the thin layer through CT without breath Temporarily, determine the best level and point of piercing, the path is vertica- ling into the cysts. Hardening agents are all 99.7% ethanol, exhausted the allantoic fluid, pumped into pure ethanol and ex- hausted it after 5 - 10 minutes. Results Puncture was successful in 126 cases treated by the new methods, the cure rate was 91%, effective rate was 100%. 56 cases were treated by the old methods,the successful rate of piercing was 89. 3% ,the cure rate was 89.3 %, effective rate was 100%. Conclusion The positioning of the improved technology is accurate, the successful rate of piercing improved Significantly ,the healing is Guaranteed, the improved technology is worth promoting in the clinical application.
