经阴道与经腹部超声对宫外孕早期诊断的临床应用比较

目的 评价两种方法对宫外孕早期诊断的临床运用价值.方法通过经阴道超声（A组）或经腹部超声（B组）检查诊断,再经腹腔镜和手术病理证实,比较两种方法对宫外孕早期诊断的符合率和超声表现检出率.结果在612例病例中,A组对附件肿块、卵巢、卵黄囊、胚芽和心管搏动的检出率分别为97.7%、96.4%、36.4%、35.1%、29.1%,B组分别为84.8%、86.5%、12.3%、22.9%、13.3%,A组均高于B组（P＜0.01）.A组诊断结果与临床（包括手术和病理）诊断符合率为97.0%（293/302）,B组诊断结果与临床诊断符合率为83.6%（259/310）.A组高于B组（P＜0.01）.结论宫外孕的早期诊断,经阴道超声较经腹部超声能更早、更准确,超声图像更清晰,而且不受肠道气体的干扰,不受肥胖和膀胱充盈量的限制.

The clinical value of transvaginal and transabdominal ultrasonography on early diagnosis of ectopic pregnancy

Objective To evaluate the clinical value of two methods on early diagnosis of eetopic pregnancy. Methods By way of the diagnosis of Transvaginal ultrasonography （ group A ） or Transabdominal ultrasonography （ group B） and by way of the confirm by the laparoscope and operative pathological specimen, the accuracy early diagnosing rate and the detecting rates of ultrasound image in two groups were compared. Results In 612 cases,the detecting rate of A group discove- ring adnexal mass, ovary, vitelline sac, pangen and fetal heart beat are respectively 97.7%, 96.4% , 36.4%, 35.1% , 29. 1%. B group are respectively 84. 8% , 86.5% , 12.3% , 22.9% , 13.3%. The accuracy diagnosing rate of A group was 97% （293/302） ,but B group was 83.6% （259/310）. A group was higher in the detecting rate and the accuracy diagnosing rate than B group（ P 〈 0. 01 ）. Conclusion In early diagnosing ectopic pregnancy, the accuracy diagnosing rate of the early diagnosis and the ultrasound image quality clear of A group was superior to B group. A group was not influenced by the bowel gas and was not limited by the plethosomy and the filling of bladder.
