四肢肌内异位腱鞘囊肿的MRI影像与病理分析

目的:分析四肢肌内异位腱鞘囊肿的MRI影像与病理征象,提高其MRI诊断准确性。方法:回顾分析33例(35个病灶)经我院病理证实的四肢肌内异位腱鞘囊肿的临床和MRI资料。结果:33例四肢肌内异位腱鞘囊肿中,膝部24例(2例双侧发病)、肘部4例、腕部3例、踝部2例,均呈长T1长T2信号,STIR呈高信号,未侵犯邻近骨骼与关节。其中10例病灶见边缘"云絮"状或"花边"状水肿信号,27例病灶内见低信号分隔带;钆喷替酸葡胺(Gd-DTPA)增强,整个病变无强化。结论:光整无强化的囊样病灶及病灶内均匀一致的窄飘带状无强化低信号分隔带是四肢肌内异位腱鞘囊肿的特征性表现。

MRI Imaging and pathological result of ectopic ganglion in limb muscle

Objective: To discuss the MRI imaging and pathological results of ectopic ganglion in limb muscle and improve the diagnostic accuracy of MRI.Methods: The clinical data and MRI imaging results of 35 cysts in 33 cases with ectopic ganglion in limb muscle confirmed by pathological analysis in our hospital analyzed.Results: There were 24 cases with spot in knees(including bilateral symmetry in 2 cases),4 cases in elbow,3 cases in twist,2 cases in ankle,and all had hyperintensity signal on T1W and T2W and T2FLAIR,without any signs of degenerative joint and bone diseases.Ten cases showed that lesions had "wool" edge or "lace-like" edema signal,and another 27 cases showed hypointensity separations,that had no enhancement in Gd-DTPA enhanced MRI.Conclusions: MRI scanning shows no enhancement in the cysts and the uniformity narrow float ribbon hypointensity separations are the characteristic features of ectopic ganglion in limb muscle.
