经颅多普勒超声对原发性帕金森病和血管性帕金森综合征检查的临床意义

摘    要:目的探讨经颅多普勒超声(TCD)对原发性帕金森病(IPD)和血管性帕金森综合征(VPS)的临床应用价值和意义。方法对81例IPD和32例VPS患者的TCD和颅脑CT的检查结果进行比较研究。结果IPD组81例中总异常血管支数为223支(30.59%);VPS组32例中总异常血管支数为163支(56.60%);异常形式两组均以左侧大脑中、后动脉血流速度减慢为主要特点。结论IPD和VPS患者均伴有脑部血流动力学改变,VPS组的异常率高于IPD组,异常特点是左侧血流速度显著慢于右侧。

Abstract:Objective:To explore the clinical significance of transcranial Doppler(TCD)evaluated on the patients with idiopathic Parkinson's disease(IPD)and vascular Parkinson's syndrome (VPS). Methods:81cases with IPD and 32 cases with VPS were evaluated by TCD. Resultls:The number of abnormal vascular branches was 223( 30.59% )among the arteries in IPD and the abnormal branches were 163(56.60%) for the arteries in VPS.The abnormality pattern was characterized by slowed blood flow in the middle and posterior cerebral arteries in the left hemisphere. Conclusion:The result indicated that there were significant changes in the hemodynamics of cerebral blood flow in patients with IPD and VPS.The abnormality rate of artery branches were higher in patients with VPS was higher than that of IPD,and the abnormality was featured by much slower blood flow in the arteries in the left hemisphere than in the riyht.
