白肾征在输尿管梗阻中的诊断价值

目的：探讨自肾征的形成机理在输尿管梗阻中的诊断价值。方法：对静脉肾盂造影（IVP）显示白肾征象的患者42例进行临床分析。结果：29例为阳性结石，9例为阴性结石，3例为血凝块引起的输尿管的急性或亚急性完全梗阻，腹膜后转移瘤1例；IVP表现为随造影时间的延长，肾实质密度持续增高（即“白肾”）。结论：白肾的意义在于提示输尿管梗阻尚处于急性或亚急性期，而肾功能尚未明显受损，提请泌尿科医生及时予以解除，以免延误治疗时机造成肾功能损害。

Value of white kidney sign in diagnosing urethral obstruction

Objective：To study the mechanism of white kidney sign and to evaluate the value of diagnosis in urethral obstruction . Methods：42 cases of whiter kidney sign confirmed by IVP were clinically analyzed. Results：In 42 cases,there were 29 with nonradiolucent calculus,9 with radiolucent calculus and 3 with blood coagulum caused acute or subacute complete urethral obstruction, one with retroperitoneal metastasis.On IVP,the density of the renal parenchyma shadow became higher with time extending （so called white kidney）. The renal function was recovered quickly and white kidney sign disappeared on IVP,when ureteral calculus were expeled in 10 cases after one week.Conclusion：The X-ray sign of white kidney significantly suggests that the urethral obstructionis stiU in acute or subacute stage while the renal function has not been damaged obviously,so that the urologist should be remined to release the obstruction in time to prevent the damage of renal function caused by delayed treatment.
