慢性肾衰竭伴高血压患者与原发性高血压患者颈动脉及眼动脉的高频超声对比研究

摘    要:目的:应用高频超声检测慢性肾衰竭伴高血压患者与原发性高血压患者颈动脉弹性及眼动脉血流动力学变化。方法:30例慢性肾衰竭伴高血压患者、20例原发性高血压患者及20例正常人(对照组)应用瞬时波强技术(wave intensity,WI)检测颈总动脉僵硬度(β)、压力应变弹性系数(Ερ),并且应用e-flow技术检测眼动脉收缩期峰值流速(PSV)、舒张末期血流速度(EDV)、平均血流速度(Mn V)、阻力指数(RI)。结果:与对照组比较,慢性肾衰竭伴高血压组与原发性高血压组RI、β、Ερ均高于对照组(P<0.05),而PSV、EDV、Mn V均低于对照组(P<0.05);与原发性高血压组比较,慢性肾衰竭伴高血压组的β、Ερ明显增高(P<0.05),而眼动脉RI、PSV、EDV、Mn V差异无统计学意义(P>0.05)。结论:高频超声可作为评价慢性肾衰竭伴高血压患者颈动脉血管早期损害及眼动脉血流动力学的技术。

Abstract:Objective:To detect the arterial elasticity of common carotid artery (CCA)and the hemodynamic changes of the ophthalmic artery(OA) in chronic renal failure(CRF)patients with hypertension in comparison with primary hypertension patients by high-frequency ultrasound.Methods:30 patients with CRF,20 patients with primary hypertension and 20 control subjects were recrui-ted.Stiffness index(β),press-wave elastic modulus(Ερ),peak systolic velocity(PSV),peak diastolic velocity(EDV),mean velocity (MnV) and resistant index(RI) were obtained at the level of CCA by wave intensity technique(WI) and at the level of OA by e-flow technique.Results:Compared with the control group,β and Ερ were higher in patients in the CRF group and the primary hypertension group(P<0.05),but PSV,EDV and MnV were lower in patients with the CRF group and the primary hypertension group(P<0.05). Compared with the primary hypertension group,β and Ερ were higher in the CRF group(P<0.05) and there was no significant differ-ence in RI,PSV,EDV and MnV(P>0.05).Conclusions:High-frequency ultrasound is a tool for evaluating early damage of CCA and the hemodynamic changes of the ophthalmic artery in CRF patients with hypertension.
