874例女性胃癌的临床分析

目的探讨女性胃癌临床病理特点、治疗模式和疗效。方法回顾874例女性胃癌与同期l737例男性胃癌的特点。结果女性胃癌发病率占同期胃癌的33．47％。与同期男性胃癌相比，其发病年龄较早；病理类型中低分化癌、黏液腺癌及印戒细胞癌等分化较差的类型占总数的52．06％，差异有统计学意义（P〈0．001）。本组病例总的1、3、5年生存率分别为52．45％、31．83％、29．74％，根治性切除术组1、3、5年生存率均明显高于其它组，差异有统计学意义（P〈0．001）；同期男性胃癌病例总的1、3、5年生存率分别为53．04％、38．19％、31．39％，男性及女性组无显著统计学差异（P=0．8224〉0．05）。结论女性胃癌发病年龄较轻，细胞分化差，恶性程度高，易发生脏器侵犯，但与男性胃癌在总体生存上无显著性差异。

Clinical analysis of 874 cases of gastric carcinoma in females

Ojective To investigate the clinicopathological features, thepeutic approaches and the prognosis of gastric carcinoma in females. Methods 874 cases of female and 1 737 cases of male gastric carcinoma were analyzed retrospectively based on their pathological types, staging, operations and 5-year survival rate. Results 874 cases of female gastric carcinoma with a morbidity of 33.47% in whole cases, and occurred in earlier age than that of male cases. The ratio of poorly differentiated carcinoma (52.06%) is significantly higher than that of the male cases (P < 0.001). 1-, 3-, and 5-year survival rate was 52.45%?31.83 %?29.74% respectively, and the 1-, 3- and 5-year survival rate of the group with radical resection was significantly higher than that of other groups(P < 0.001). 1-, 3-, and 5-year survival rate of the male cases was 53.04%?38.19%?31.39% respectively, there is no significant difference in the 5-year survival rate between the male and the female cases(P = 0.8224 > 0.05). Conclusions Female gastric carcinoma occurred in an earlier age, with poorer differentiation, higher malignant degree and higher occurrence rate of adjacent invasion. However, there is no significant statistical difference in 5-year survival rate between male and female cases.
