多普勒超声对婴幼儿肠套叠的诊治优势及应用价值

目的 通过彩色多普勒超声对小儿肠套叠进行诊断与超声监视下生理盐水灌肠复位的研究,探讨超声诊断与协助治疗该病的价值.方法 采用东芝8000型彩色多普勒超声诊断仪,对2007年1月～2009年1月在我院门诊确诊的小儿肠套叠患儿150例,运用高频多普勒超声诊断仪,观察肠套叠征象.结果 多普勒超声及超声引导下水压灌肠诊治价值远大于腹部平片,其超声表现为：局部包块横切面为同心圆征、纵切面为套筒征、肠管内多有增大淋巴结回声、肠管间多有少量积液等,同时通过观察彩色多普勒显示肠管壁的血流情况可判断肠管的血液情况.结论 多普勒超声诊断小儿肠套叠准确率高、可靠,超声监视下水压灌肠安全性高、治愈效果好,其治疗效果比X线监视下空气灌肠复位有明显优势,而且可避免不必要的X线照射,值得临床推广应用.

The Advantages of Doppler Ultrasound for Infants Intussusception's Diagnosis and Treatment

Objective Via treating infants intussusception diagnosed by ultrasonnd with normal saline enema under uhrasnund surveillanee to explore ultrasound for its diagnosis and treatment. Methods One bundred and fifty infants with intussusception diagnosed by Toshiba 8000 was selected from January 2007 to January 2009 and its signs was observed using high-frequency doppler ultrasound diagnostic apparatus. Results The advantages that the diagnosis of infants intussusception by doppler ultrasound and its treatment under ultrasound sur- veillance is far greater than that of the plain abdominal radiogTaph. Its ultrasound signs were as follows ：The partial mass appeared as concen- tric circle Sign on cross-sectional images ,and sleeve Signs on hmgitudinal section images;there were swelling lymph nodes in intestinal canal; small amounts of fluid was found between intestine. The blood flow of the intestinal wall was observed by color Doppler so as to judge the blood supply of the intestine,which is important for clinicians to determine the treatment protocols. Conclusion Doppler ultrasound diagnosis of infants intussusception is accurate and reliable. Its treatment with normal saline enema under the ultrasound surveillance has high security, and the healing effect is better than that of gaseous enema reduction under X-ray. This treatment is worth promoting in clinic.
