无张力疝修补术在降低老年人腹股沟嵌顿疝术后肺部感染中的作用

目的探讨老年人腹股沟嵌顿疝术后并发肺部感染的原因,提出防范措施。方法回顾性分析我院86例70岁以上腹股沟嵌顿疝急诊手术病人的临床资料。结果采用无张力疝修补术的29例术后只有1例并发肺部感染,行传统的疝修补术的57例术后有23例并发肺部感染。结论慢性支气管炎和肺气肿、吸烟史及传统的手术方式是术后并发肺部感染的三项高危因素。采用无张力疝修补术以及加强呼吸道护理和抗感染治疗是预防和治疗老年人腹股沟嵌顿疝术后肺部感染的有效方法。

Analysis of postoperative pulmonary infections in elderly inguinal strangulated hernia

ObjectiveTo analyze the causes of postoperative pulmonary infection in elderly patients with inguinal strangulated hernia.MethodsThe data of 86 patients more than 70 years old in our hospital were retrospectively analyzed.ResultsAmongst these 29 cases who were treated with tension-free hernioplasty,postoperative pulmonary infections occurred in one of them.Amongst these 57 cases who were treated with traditional hernioplasty,postoperative pulmonary infections occurred in 23 of them.Patients with tension-free hernioplasty had significantly lower morbidity rate than those with traditional hernioplasty(P<(0.01)).ConclusionThe most dangerous factors include chronic bronchitis and pulmonary emphysema,smoke,traditional operative procedure.Tension-free hernioplasty,respiratory tract nurse,anti-infective treatment are effective for preventing and treating postoperative pulmonary infections in elderly patients with inguinal strangulated hernia.
