益气活血解毒法治疗冠心病PCI术后心绞痛临床体会

目的：观察益气活血解毒法治疗经皮冠状动脉介入术（PCI）后心绞痛的临床疗效。方法：将60例PCI术后心绞痛患者随机分为对照组和治疗组，每组30例，对照组采用西医常规治疗，治疗组在西医治疗基础上加用自拟益气活血解毒汤治疗。均以8周为1个疗程。观察两组治疗前后心绞痛症状及心电图改变，于治疗开始前1天与结束后次日空腹抽取静脉血，测定血脂水平及c反应蛋白（CRP）含量。结果：治疗8周后，治疗组心绞痛总有效率为90．0％，优于对照组的70．0％（P〈0．05）。治疗组改善心电图疗效为53．3％，优于对照组的30．0％（P〈0．05）。治疗组可明显降低患者血清总胆固醇（TC）浓度同时升高血清高密度脂蛋白总胆固醇（HDL—C）浓度（P〈0．01，P〈0．05），可明显降低血清CR．P含量（P〈0．01）；对照组亦可降低血脂及CILP含量，但无统计学意义。与对照组治疗后比较，治疗组可明显降低患者血清TC浓度及CILP含量，并升高血清HDL—C浓度（P〈0．05）。结论：益气活血解毒法配合西药常规治疗能明显缓解PCI术后心绞痛症状，调节血脂和抑制炎症反应可能是其作用机制之一。

Clinical Experience about Chinese Herbs with Effects of Replenishing Qi and Activating Blood Circulation and Detoxication Preventing on Angina after Percutaneous Coronary Intervention

Objective ： To observe the clinical effects of Chinese herbs with effects of replenishing qi and activating blood circulation and etoxication preventing on angina after Percutaneous Coronary Intervention （ PCI ）. Methods ： 60 patients with angina after PCI were randomly divided into control group treated with basic medicine and treatment group treated with Chinese herbs with effects of replenishing Qi and activating blood circulation and etoxication decoction with basic medicine, 30 cases each group, and treated for eight weeks. To observer the changement of angina, ECG, and measure concentrations of serum lipid and C reaction protein （ CRP ）. Results ： After being treated for eight weeks, the total effective rate of 90.0% in treatment group about angina was obviously better than control group with 70. 0% （P〈0.05）.The total effective rate of 53.3% in treatment group on ECG was obviously better than control group with 33.3% （P〈0.05）. The treatment group significantly reduced the concentration of total cholesterol （ TC ） and increased the concentration of high density lipoprotein cholesterol （HDL-C） （P〈0.01, P〈0.05 ）, also significantly reduced the concentration of CRP. The control group also reduced the concentration of serum lipid and CRP, but the differences were not significant （P〉0.05） .Comparing with the control group, the treatment group significantly reduced the concentration of serum lipid and CRP, increased the concentration of HDL-C （P〈0.05）. Conclusion ： Chinese herbs with effects of replenishing Qi and activating blood circulation and etoxication with basic medicine can significantly relive angina after PCI, probably one of the mechanisms of that decoction depends mostly on regulating the serum lipid and preventing endothelial inflammatory reactions.
