腭板植骨术后5年腭咽闭合功能分析

目的 :探讨腭板骨缺损修复对腭咽闭合功能的影响。方法 :腭板骨缺损的先天性Ⅱ°腭裂患者 4 0例 ,随机分为对照组和植骨组 ,植骨组腭板骨缺损利用HA -Bonecement复合人工骨修复 ,术后 5年复诊 ,所有患者行鼻咽纤维镜腭咽闭合功能定量对比分析检测 ,植骨组患者加腭部CT扫描检查。结果 :植入的人工骨在植骨床固定牢固 ,未见移位、未见塌陷、未见断裂 ;植骨组软腭活动度和腭咽闭合的比例明显高于对照组 ,但术后 5年腭咽闭合功能定量分析结果与术后 3个月比较没有统计学意义。结论 :腭板骨缺损修复远期效果显示能改进腭咽闭合 ,但腭板骨缺损修复改进腭咽闭合的程度随植骨时间的延长无明显提高

An Analysis of Velopharyngeal Function about Repairing the Hard Palate Cleft with the Composited Artificial Bone after Five Years

Objective:This study was to analyze velopharyngeal function about repairing the hard palate cleft with the composited artificial bone after five years.Methods:40 patients with hard palate cleft were randomly divided into two groups:the implantation group and control group.Each group had 20 patients.The patients in the implantation group were implanted with composited artificial bone of HA-Bone cement .All patients had been followed up for five years postoperatively. The patients in the implantation group were examined using computer tomography.All patients were examined by NPF.Results:The implanted artificial bone in hard palate had no shifting ,no subsidence and no breakage. the movement of soft palate of implantation group was greater than the control group . The velopharyngeal function of the implantation group was much better than the control group.But there was no obvious difference on the velopharyngeal function of the implantation group for five years postoperative and for three months postoperative.Conclusions:Repairing the hard palate cleft with the composited artificial bone can improve velopharyngeal function.But the velopharyngeal function can not obvious improve with the time.
