中美联合院前急救培训课程培训效果分析

摘    要:目的:评估中美联合院前急救培训课程的培训效果,进一步优化培训方法,促进中国院前急救培训的标准化与规范化发展。方法:联合培训组选取参加2010年度中美联合院前急救培训课程的浙江省院前急救医生共113名,采用专门编写的《浙江省院前急救知识与技能规范化培训教材》和以模拟情景教学方法为主的综合教学方法,就"理论知识"、"止血+包扎+固定+搬运"、"气管插管"、"心肺复苏"等方面进行授课。中美双方师资配比为1∶2,理论与技能培训时间比为1∶2。培训前后进行理论与技能考核,考核后对所有学员进行课程评估问卷调查。对照组选取同期参加其它地市急救中心举办的院前急救培训班学员共85名,培训教材选用本科教材《急诊医学》,培训师资由中方院前急救高年资医生组成,培训方法以除模拟情景教学方法以外的其它综合教学方法,培训内容、时间、考核方法等同联合培训组。t检验比较每组学员培训前后得分与两组学员培训前后得分差值的差异,以P<0.05为差异具有统计学意义。结果:每组学员培训后理论知识与各项技能得分均明显优于培训前。联合培训组学员的理论知识、止血+包扎+固定+搬运、气管插管等项目得分优于对照组(P<0.05),心肺复苏与心律失常处理项目两组得分无显著差异(P>0.05)。结论:中美联合培训课程能显著提高中国院前急救医生对急救知识与技能的掌握,尤其在理论知识、止血+包扎+固定+搬运、气管插管等项目培训效果优于传统培训方法,但对心肺复苏与心律失常处理等项目无明显优势。

Impact of the China-America union pre-hospital emergency training program on the emergency knowledge and skills of Chinese first-aid doctors

Abstract:Objective To evaluate the effect of the China-America union pre-hospital emergency training program on the first aid doctors and optimize the training methods to develop and standardize the pre-hospital training procedure in China.Methods A total of 113 pre-hospital emergency doctors who participated into the Union Training Program in 2010 were included into the union training group.Training materials about pre-hospital emergency knowledge and skills and the integrated teaching methods including scenarios teaching were adopted.The contents of the training course included the emergency knowledge,bleeding treatment,bandage,fixation,transportation,intubation and CPR and so on.The ratio of teachers’ number between Chinese and America was 1∶2,the ratio of training time between knowledge and skills was 1∶2.All trainees were tested about the emergency knowledge and skills and also were surveyed about the evaluation of the program.Another 85 pre-hospital emergency doctors who took part into the other traditional first aid training course launched by municipal Emergency Center were selected into the control group.The textbook for the medical students,Emergency Medicine,was taught by Chinese pre-hospital emergency doctors in the class.The methods and content and time of the training were the same as the union training group’s except the scenarios teaching way.T-test was used to observe the difference of scores of students before and after training in each group and between the two groups after training.P<0.05 was considered statistically significant.Results The trainees’ scores of both knowledge and skills were much higher after the training than those before the training in each group.The scores of the students in the union training program after the training in the emergency knowledge,bleeding treatment,bandage,fixation,transportation and intubation were much higher than those in the control group(P<0.05).And the difference of the scores in CPR and arrhythmia treatment in the two groups was not statistically significant(P>0.05).Conclusion The union training program can improve the Chinese pre-hospital emergency doctors’ requirement of emergency knowledge and skills,especially in the aspects of knowledge,bleeding treatment,bandage,fixation,transportation and intubation.But the training result in the CPR and arrhythmia treatment is of no obvious priority.
