CT引导下经皮肺穿刺对肺周围性结节病变的诊断价值

目的 探讨CT引导下经皮肺穿刺活检病例在肺外周病变诊断中的价值。方法 选取2005年3月～2006年3月在我院住院治疗经放射影像学、脱落细胞学、细菌学及电子支气管镜检查均不能明确诊断的肺部外周性病变患者51例，在CT定位引导下使用槽式切割活检针进行病理学检查。结果 本组47例获明确诊断，组织学诊断率为92．2％，一次穿刺成功43例，两次穿刺成功8例；术后并发症发生率9．8％，其中气胸3例，局部出血1例，咯血1例，经相应处理后缓解。结论 CT引导下经皮肺穿刺是临床诊断肺部周围性病灶的一种重要辅助诊断方法。对良、恶性病变的敏感度、特异度较高，并发症少，安全有效。CT引导下经皮肺穿刺阴性结果主要与病灶较小、病变不典型有关。

The diagnostic value for pulmonary peripheral occupying lesion by percutaneous puncture biopsy under CT guided

Objective To explore the diagnostic value for pulmonary peripheral occupying lesion by percutaneous puncture biopsy under CT guided.Methods 51 patients with pulmonary peripheral lesion were selected for the study They were inpatients cared in our hospital between March of 2005 and March of 2006 without identified diagnosis by radio-imageology,cytology,bacteriology and bronchoscopy.The pathological diagnosis was performed after getting the tissue by the groovy needle punctured the lesion under CT guided.Results The total 47 patients had defined pathological diagnosis,and its histodiagnosis rate was 92.2%.Among them,43 patients had only one-time puncture,8 patients had two-time punctures.The incidence post-biopsy was 9.8%(3 pneumothorax,1 hymoptysis,1 region bleeding),all relieved after treatment.Conclusion(1)It is the important to use the auxiliary diagnosis method with diagnosis pulmonary peripheral occupying lesion by percutaneous puncture biopsy under CT guided.(2)The method has a high rate for diagnostic sensitivity and for accuracy,which may be used for the clinical diagnosis and treatment of the disease.(3)The method is relatively safe and effective as a diagnostic technique.(4)The negative results by using the method are mostly related to small and atypical lesion.
