| 摘 要： | 目的探讨N末端脑钠肽前体(NT-proBNP)在川崎病(KD)患儿中的变化及临床意义和可能机制。方法分别测定38例KD急性期患儿和30例正常体检小儿血浆NT-proBNP水平,同时测定外周血WBC、ESR、CRP及血浆心肌肌钙蛋白(cTnT)水平,常规超声心动图检测左室射血分数(LVEF)、二尖瓣舒张早期流速峰值/二尖瓣舒张晚期流速峰值(E/A)及冠状动脉病变情况。结果 KD急性期患儿血浆NT-proBNP、WBC、CRP、ESR、cTnT水平均明显高于健康对照组(P<0.05,P<0.01)。通过直线回归分析,KD急性期患儿血浆NT-proBNP水平与LVEF、E/A无相关性(r=0.56,r=1.20,P>0.05);而与WBC、CRP、ESR呈显著正相关(r=1.25、1.82、1.60,P<0.01),与患儿cTnT水平亦呈正相关(r=1.95,P<0.05)。结论血浆NT-proBNP水平在KD急性期显著升高,可能与KD患儿急性期心肌炎性反应及炎性因子的升高有关。
