前路、后路手术治疗合并颈椎管狭窄的单节段脊髓型颈椎病

摘    要:目的:比较前路和后路手术治疗合并颈椎管狭窄的单节段脊髓型颈椎病的疗效。方法:手术治疗合并颈椎管狭窄的单节段脊髓型颈椎病31例,前路减压植骨融合术(A组)19例,后路单开门椎板扩大成形术(P组)12例。采用JOA评分比较两组术后疗效,t检验进行统计分析。结果:A组术后平均改善率为75.9%,P组为64.8%,两组间术后疗效无统计学差异(P>0.05)。结论:手术治疗合并颈椎管狭窄的单节段脊髓型颈椎病,前路和后路两种术式均可获得满意疗效。

Abstract:Objective To compare the therapeutic effect of anterior and posterior operations in the treatment of one-level lesion of cervical myelopathy with spinal canal stenosis. Method 31 patients of one-level lesion of cervical myelopathy with spinal canal stenosis were treated with surgery. 19 patients (group A) were performed anterior cervical discectomy and fusion (ACDF), and 12 patients were performed laminoplasty without resection of the herniated disc (group P). The severity of myelopathy was evaluated with the Japanese Orthopaedic Association's scoring system(JOA) and surgical outcomes were evaluated by the system of Hirabayashi for determining the recovery rate. Results The average recovery rate is 75.9% in group A and 64.8% in group P. There is no significant difference between the two groups(P>0.05). Conclusion Both anterior and posterior procedures can obtain good results of neurologic recovery in the treatment of one-level lesion of cervical myelopathy with spinal canal stenosis.
