基层医院咪达唑仑复合丙泊酚用于无痛胃镜检查中的疗效观察

目的：探讨咪达唑仑复合丙泊酚静脉麻醉用于无痛胃镜的临床效果及安全性。方法：收集本院无痛胃镜检查的患者共300例，随机分为观察组（A组）与对照组（B组），A组在异丙酚1.5～2.5mg/kg+咪达唑仑0.01～0.05mg/kg，B组单用异丙酚1.5～2.5mg/kg麻醉。比较两组患者的术前及给药后的HR、MAP、SPO2；苏醒时间；异丙酚用量的差异，术后询问患者的遗忘程度及满意度。结果：300例患者均完成胃镜检查，安全离院。与A组相比，B组的患者的遗忘程度及满意度上无明显差异（P>0.05），但B组生命体征的波动小于A组（P<0.05），且苏醒时间长于A组有统计学意义（P<0.05），丙泊酚的用量明显少于A组有统计学意义（P<0.05）不良反应B组也明显多于A组有统计学意（P<0.05）。结论：咪达唑仑复合丙泊酚应用于胃镜检查是一项安全、可靠、无痛的检查方法，值得临床推广。

Clinical observation On propofol combined with midazolam in painless gastroscopy

Objective :To explore the effect,safety and value of propofol combined with midazolam in painless gastrointestinal endoscopy. Methods: 300 patients of our hospital were collected and divided into group A (n=150 ,infusion with midazolam 0.01~0.05mg/kg +infhsion with propoful1.5~2.5mg/kg 5 min later) and group B (n=150, infusion with infhsion with propoful 1.5~2.5mg/kg),The mean anterial pressure(MAP),heart rate(HR)and pulse oxygen satufation(SpO2)were obtained before infusion,aner disappearance of eyelash reflex, and the time for regaining eonsciousness, propofol dosage, and were also retcorded..Results:300 case of patients were able to complete the gastrointescopy examination successfully with no serious side effect,Compared with group A, The patients of group B were no significant diffent in satisfaction and degree of forgetting (P>0.05),But the MAP、HR、SpO2 is chang more obvious in group A (P<0.05), the eyeopening time were longer in group B(P<0.05), adverse reaction were lesser in group B(P<0.05)and the consumed dose of propofo1 were lower in group B (P<0.05).Conclusion:Propofol combined witIl midazolam in painless gastrointescopy was a safe,effective and painless ,process and was worthy of promoting widely.
