摘    要:目的:比较不同外形纤维桩桩核和粘结系统对前牙残冠残根桩核修复疗效的影响。方法:选取前牙残冠残根患者90例,共102颗患牙,按使用桩核外形不同分为两组,分别采用Tenax fiber white(T组)纤维桩和Parapost fiber lux(P组)纤维桩。各自再均分为两组,采用不同粘结系统进行桩核修复及常规烤瓷冠修复。随访观察1-2年,统计各组残根残冠的治疗成功率并进行统计学分析。结果:T组和P组的治疗成功率分别为85.4%和88.9%,组间差异无统计学意义CP=0.59)。13例失败患牙中,采用全酸蚀系统失败3颗牙,自粘结系统失败10颗牙,经x^2检验,不同粘结系统可以显著影响残根的预后疗效(P〈0.05)。结论:修复前牙残冠残根时,应根据根管情况选择合适的纤维桩,全酸蚀系统成功率优于自粘结系统。

Abstract:Objective: To evaluate the effect of post shape and bonding systems on the survival of residual crown and roots. Method: 102 residual roots were chosen to restore and were divided into two groups with Tenax fiber white post (T group) and Parapost fiber lux post (P group). In each group, two bonding systems were both used. After dowel restoration, all teeth were restored with resin core and ceramic crowns. The success rate of prostheses was observed after 1 to 2 years and the data were analyzed by x2 test. Result: The success rates for T group and P group were 85.4 % (41 / 48) and 88.9 % (48 / 54) respectively, and there was no significant difference between two groups. However, among 13 failed teeth, 3 teeth were with the total etch system while 10 with the self-adhesive system. Kind of bonding systems significantly affected the survival of residual roots (P 〈0.05). Conclusion: It is alternative to restore residual crown and roots with either shape of glass fiber post depending on the roots enviroment and total etch system will significantly increase the success rate of pulpless residual roots compared with the self-adhesive system.
