诺扬在门诊老年患者无痛肠镜检查应用的可行性观察

目的观察诺扬在老年患者无痛肠镜检查中的应用。方法 120例ASAⅠ～Ⅱ级无痛肠镜检查老年患者（〉65岁）,随机分为两组：芬太尼组（F组）和诺扬组（N组）,比较两组推注得普利麻前（T0）及后1min（T1）、2min（T2）、3min（T3）的血压、心率、呼吸频率以及低氧血症发生率,以及苏醒时间、离开医院时间,术后腹痛、头晕、恶心、呕吐的发生率,和24h后随访无痛检查满意度。结果两组T0、T1、T2、T3的血压、心率、呼吸频率差别无统计学意义（P〉0.05）,F组低氧血症发生率较N组高（P〈0.05）。苏醒时间、离开医院时间、恶心、呕吐的发生率两组无统计学意义（P〉0.05）,头晕发生率N组高于F组（P〈0.05）。术后腹痛发生率N组低于F组（P〈0.05）,24h后电话随访患者的满意度N组高于F组（P〈0.05）。结论在门诊行无痛肠镜检查的老年患者,诺扬复合得普利麻优于芬太尼复合得普利麻,术后腹痛发生率更低、满意度更高,不失为一种较好的选择。

Clinical study of butorphanol tartrate combined with propofol for colonoscopy in elderly patients

Objective To investigate the effect of Butorphanol Tartrate Injection combined with propofol for colonoscopy in elderly patients.Methods Sixty elderly patients patients（ASA Ⅱ～Ⅲ,65 years old） undergoing colonoscopy were recruited in this study.Patients were randomly divided into 2 groups： Fentanyl group（group F） or Butorphanol Tartrate group（group N）,Compare and analyze the blood pressure,heart rate,respiratory rate,the incidence of hypoxemia just before（T0）,at 1 minute（T1）,2 minute（T2）,5 minute（T3） between the two groups.Recovery time,time to leave the hospital,the incidence of postoperative abdominal pain,dizziness,nausea,vomiting,the rate of satisfaction with colonoscopy were also recorded.Results There was no significance of the blood pressure,heart rate,respiratory rate at T0,T1,T2,T3 and recovery time,time to leave the hospital,the incidence of nausea,vomiting between the two groups（P0.05）.The incidence of hypoxemia and postoperative abdominal pain,in group F was significantly higher than that of group N（P0.05）,but the incidence of dizziness and the rate of satisfaction with colonoscopy were lower that of group N（P0.05）.Conclusion The effect of Butorphanol Tartrate Injection combined with propofol was better than Fentanyl combined with propofol for colonoscopy in elderly patients.
