腹腔镜联合米非司酮治疗异位妊娠的疗效评价

目的比较腹腔镜手术与开腹手术联合米非司酮治疗异位妊娠的疗效。方法回顾性分析笔者所在医院收治的异位妊娠患者122例，随机分为两组，其中腹腔镜组60例，开腹组62例，术后两组均口服米非司酮50mg，连服5d。比较两组患者手术时间、住院时间、术中出血量、术后肛门排气时间、镇痛药应用等近期疗效，以及术后HCG值降低至正常水平的时间、症状消失时间、恢复工作情况和持续性异位妊娠的发生率等远期疗效。结果腹腔镜组患者近期疗效显著优于开腹组患者（均P〈0．05），两组患者β-HCG值降低至正常所需时间及持续性异位妊娠的发生率未有显著差异（P值分别是0．625和0．874），腹腔镜组患者症状消失时间及恢复工作时间显著优于开腹组（P值分别是0．013和0．037）。结论腹腔镜联合米非司酮治疗异位妊娠有很好的近期和远期疗效，并能很好的预防持续性异位妊娠的发生。

Study on the effect of laparoscopic united mifepristone treatment to ectopic pregnancy

Objective To compare the effect of laparoscopic and laparotomy united mifepristone treatment to ectopic pregnancy. Methods 122 cases of patients with ectopic pregnancy were randomly divided into two groups, with 60 cases in the laparoscopic group and 62 cases in the laparotomy group. After surgery both groups of patients oraled mifcpristone 50 mg for 5 days. To compare the short - term efficacy such as operation time, hospitalization time, blood loss volume, anal exhaust time and the recent application of analgesic and long - term efficacy such as the time of β - HCG reduced to normal levels, the time of symptoms disappeared and back to work, and the incidence of persistent ectopic pregnancy between the two groups. Results The short - term efficacy in the laparoseopic group was significantly superior to that in the laparotomy group （P 〈 0. 05 ） ,the time of β - HCG reduced to normal levels and the incidence of persistent ectopic pregnancy in both groups were of no significant difference （P = 0. 625,0. 874, respectively）, the time of symptoms disappeared and return to work in the laparoscopie group were significantly better than those in laparotomy group group （P =0. 013,0. 037 ,respectively）. Conclusion Laparoscopie united mifepristone treatment to ectopie pregnancy had a very good short - term and long - term efficacy and prevented the occurrence of persistent ectopic pregnancy.
