保留部分脾脏在肝硬化巨脾症中的应用

目的：研究肝硬化巨脾症行脾大部切除保留部分脾脏的意义.方法：采用随机分组，对肿大部切除（研究组）及脾切除（对照组）进行血细胞、免疫指标及促吞噬激素（Tuftsin）的对比研究，并对手术前后自由门静脉压（EPP）、术后情况进行分析.结果：术后对照组的白细胞、血小板计数较研究组提高明显（P＜0.05～0.001）;研究组的lgA、IgM，Tuftsin术后明显高于对照组（P＜0.01～0.001）;术后两组FPP改变无差异，对照组术后持续发热例数较多.结论：两组术后均能降低门静脉压力.保留部分脾脏较对照组有明显改善机体体液免疫功能，以避免血液粘稠度一过性骤升，在抗感染过程中发挥了重要作用.

Application of Retaining Part Spleen in Cirrhotic Megalosplenia

Objective: To study the significance of retaining part spleen in splenectomize of cirrhotic megalosplenia. Methods: Patients were divided randomly into partial splenectomized group (experiment group) and splenectomized of cirrhotic megalosplenia (control group). Comparahve study was made in blood cell. inununological index and tuftsin. Free portal vein pressure (FPP) was analyzed in pre-operation and post-operation. Results: White blood cell and platelet counts of control groups were significantly higher than those of experiment groups in post-operahon (p<0.05-0.001).Values of IgA, IgM and tuftsin of experiment groups were significantly higher than more those of control groups in post-operation (P<0.01-0.001).There was no difference in FPP of two groups. More cases of fever were in control groups in post-operation. Conclusions: Post-operative FPP was lower in two groups. Retaimng part spleen could significantly improve human immunological functions more than control group and avoid instantaneous higher in blood sticky degree, it also could play an important role in anti-infection Process.
