癫痫儿童典型失神发作与中央颞区棘波的共存现象

目的：分析失神癫痫儿童之典型失神发作与中央颞区棘波（CTS）的共存现象，探讨典型失神发作伴CTS的脑电图（EEG）与临床特征。方法：检索10年来所有典型失神发作患儿的EEG资料，对伴有CTS病例的临床病程、EEG特征及演变过程和治疗情况进行回顾性分析。结果：共检索到72例典型失神发作患儿，其中4例在失神发作活动期或发作控制后至少有1次EEG监测有CTS，均不伴临床部分性发作，所有患儿对丙戊酸单药治疗效果好。结论：典型失神发作可与CTS共存于同一患者，二者具有各自独立的EEG与临床特征，失神发作的控制均伴有广泛性3Hz棘慢波放电的消失，而CTS常持续存在多年而不引起临床发作。对这类患儿选择广谱抗癫痫药物更好。

A concomitance of typical absence and centrotemporal spikes in children with epilepsy

Objective： To analyze the concomitant phenomenon of the typicalabsence and centraltemporal spikes （CTS） in children with childhood absence epilepsy （CAE）, and the electro-clinical characteristics of typicalabsence with CTS. Methods： The EEG data of all patients with typical CAE in the last 10 years, and retrospective anaysis of the clinical course, EEG features, evolving process and therapy conditions were carried out. Results： Four cases of the 72 children with typical CAE showed that CTS appeared at least once by EEG monitoring when absence active phase or seizures had been controlled. All of them never had corresponding partial seizures and had good response to valproic acid. Conclusion： The typical CAE and CTS can atlaek concomitantly at the same individual and both special EEG and clinical characteristic. The control of absence in all patients is concomitant with the disappeanee peananee of generalized 3Hz spikes and waves, but CTS usually exists continuously for many years without clinical seizures. The inner link within both conditions is under furtherstudy. Broad spectrum anti-epileptic drugs are better for these patients.
