显微手术治疗矢状窦旁及大脑镰旁脑膜瘤的临床疗效研究

目的:探讨采用显微外科手术切除矢状窦旁脑膜瘤及大脑镰旁脑膜瘤临床疗效。方法:对2008年4月～2011年9月采用显微外科手术切除矢状窦旁脑膜瘤及大脑镰旁脑膜瘤34例患者的临床资料进行回顾性分析。结果:本组34例患者,SimpsonⅠ级切除9例(26.5%),SimpsonⅡ级切除20例(58.8%),SimpsonⅢ级切除5例(14.7%)。9例上下肢运动或感觉障碍者,6例恢复正常,2例未能完全恢复,1例运动功能障碍加重。7例癫痫者经服药后仅2例偶有发作。2例精神障碍者均恢复正常。全部病例无手术死亡。术后随访3～38个月,2例失访,3例复发,其中2例再次手术,1例行伽玛刀治疗。结论:良好的手术暴露,熟练的显微手术技巧,矢状窦和中央沟静脉的妥善处理和保护,避免脑皮质损伤,是提高矢状窦旁及大脑镰旁脑膜瘤手术全切率和手术疗效,减少并发症的重要因素。

Microsurgical Treatment for Parasagittal and cerebral falx Meningiomas

Objective:To study micro-surgical Treatment for parasagittal and cerebral falx meningiomas curative effect.Methods:On the 2008 April ～ 2011 September microsurgical operation excision of parasagittal sinus meningioma and cerebral falx meningioma in 34 cases The clinical data were retrospectively analyzed.Results:This group of 34 patients,Simpson grade Ⅰresection in 9 cases(26.5%),SimpsonⅡresection in 20 cases(58.8%),Simpson Ⅲresection in 5 cases(14.7%).In all cases,no operation death.Postoperative follow-up ranged from 3 to 38 months,2 cases lose follow-up,and relapse in 3 cases,including 2 cases of again operation,1 cases treated by gamma knife.Conclusions:Skillful microsurgical technigue was helpful to control of intraoperative bleeding,good protection of the vein of Central sulcus,increase in the rate of total removal of the tumors,was to improve the sagittal sinus and cerebral falx meningioma operation resection rate and operation effect,An important factor in reducing complications.
