HPV不同高危亚型在宫颈腺癌、宫颈鳞癌中的表达及临床意义研究

摘    要:目的:研究HPV不同的高危亚型在宫颈腺癌和宫颈鳞癌中的表达差异,从而得出在宫颈癌的医学治疗中HPV所发挥的作用。方法:采用引物及多重型特异聚合酶链反应检测60例门诊宫颈癌患者(其中宫颈腺癌30例,宫颈鳞癌30例)在石蜡包埋组织中HPV六种高危亚型(HPV16、18、31、33、45、16/18)的感染情况,分析不同HPV高危型在宫颈癌与宫颈鳞癌中的存在意义。结果:对60例宫颈癌患者检验后发现:其包埋组织中HPV感染率分别为71.7%、80.0%;通过引物聚合酶链反应检测感染有脱漏的现象,HPV在宫颈癌中的比例较高占主要地位。HPV高危亚型存在不同的表达形式:HPV18与宫颈腺癌最具相关性,HPV16则与宫颈鳞癌密切相关,比较差异均有统计学意义(P〈0.05)。高危型HPV感染与宫颈淋巴结转移、肿块大小、临床分析、及组织学分级均无关(P>0.05)。HPV18感染与年龄及淋巴结转移关系密切(P〈0.05)。结论:HPV亚型在不同组织类型中的表达差异,在一定程度上解释了宫颈腺癌不同组织的发病机制,在临床和指导预测预后方面发挥了重要作用。

Clinical Significance of High-risk Subtype Human Papillomavirus Differential Expressions in Cervical Squamous Cell Carcinoma and Adenoearcinoma

Abstract:Objective: To study the different high-risk subtypes of HPV in cervical adenocarcinoma and squamous cell carcinoma of the uterine cervix expression, so that the medical treatment of cervical cancer in the rale of HPV.Method: 60 patients with cervical cancer outpatients ( including 30 cases of cervical adenocarcinoma, 30 cases of cervical squamous cell carcinoma ) were detected by specific primers and more severe polymerase chain reaction in paraffin embedded tissues six risk HPV subtypes ( HPV16, 18, 31, 33, 45, 16/18 ) of infection, the different high- risk HPV types in cervical cancer and cervical squamous cell carcinoma of the meaning of existence were analyzed.Result: 60 cervical cancer patients after inspection found that the organization of the embedding of HPV infection rates were 71.7% and 80.0%, by primer polymerase chain reaction detection of infection with the phenomenon of ellipsis, the higher percentage of HPV in cervical cancer.The high-risk HPV subtypes had different forms: the relationship between HPV18 and cervical cancer, HPV16 was closely associated with cervical squamous cell carcinoma, the differences were statistically significant ( P〈0.05 ) .High risk HPV infection and cervical lymph node metastasis, tumor size, clinical analysis, and histological grade were independent ( P〉0.05 ), HPV18 infection was related to age and lymph node metastasis closely (P〈0.05) .Conclusion: HPV subtypes expression differences in different tissue types, to some extent, explain the pathogenesis of cervical adenocarcinoma different organizations, in clinical play an important role in predicting prognosis and guiding clinical.
